The Department observed good temporary stabilization. Vegetation is well established. Only one of the 3 wells that have been drilled on this pad has been stimulated and placed into production, the Dayton 4H, 115-21607. The 5H, 115-22069 and the 6H, 115-22070 have been drilled only. Temporary containment is still in place around the well heads. A cage has been placed around the 4H. The pipeline tie-in has good temporary stabilization. No violations noted.

The cellar ice at the bottom, there was cement at the (13x9) annulus. There were no lines going to or from the well, all the valves were closed and a there was a TA cap on top of the well. There was a cow guard around the well. The well was drilled to TD at 13,055'. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:30p.m.

2591601 Primary Facility 2017-05-04 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m. for an inactive status request inspection.

The well was drilled to TD at 13,055' but wasn’t stimulated yet. The cellar was about half full of water with no bubbling noticed. There were no lines going to or from the well, all the valves were closed and the pressure gauges had 0psi on them. There was a TA cap on top of the well and a metal guard around the well. There were three wells on site, the 4H (115-21607), 5H (115-22069), and 6H (115-22070). The 4H is the only well that is in production. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:35p.m.

2665879 Primary Facility 2017-11-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was drilled to TD but hasn't been stimulated and was granted Regulatory Inactive Status on 5/31/17. There were no lines going to or from the well and all the valves were closed. The was cement between the (20x13) and (13x9) annuli and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2679189 Primary Facility 2018-01-05 Site Restoration No Violations Noted I inspected the Northwest Unit 11 well pad for Notice of Termination (NOT) inspection. The pad has 1 active well on site, the 4h, permit #115-21607. There are two wells, the 5h and 6h which are in Regulatory Inactive status. Their permit #s are 115-22069 and 115-22070, respectively. The site was overall in good shape with no erosional issues present at the time of inspection. The vegetation which was appearing through the snow cover appeared full. The wells(s) were properly labeled.

There were no violations observed.

I recommend APPROVAL of this NOT.

2766097 Primary Facility 2018-08-22 Routine/Complete Inspection No Violations Noted At time of the inspection.

The well was granted Regulatory Inactive Status on 5/31/17. The well was drilled to TD at 13,055' but hasn't been stimulated and was set up with a TA cap. There were no lines going to or from the well and all the valves were closed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2831786 Primary Facility 2019-01-25 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Jonathan Sites. Halliburton was on site to frac the 5H and 6H wells. They were running a plug and perf in the 6H when I arrived on site. There were 25 out of 42 stages completed on the 5H. There were 3 out of 39 stages completed on the 6H. The water was be pumped to the site from the Brant pad where fresh water only was being trucked to the site and put in tanks. I took a walk around the location and there were no obvious concerns noticed at either site. There were no violations noted.
