On 3/8/16 at 1445, Operator representative Shane Kriebel reported a >5 gallon release of brine with trace oil to SWRO representative Martha Michalek.

Per Mr. Kriebel, there was a 100 bbl plastic brine tank failure at the well site. The exact volume of the release is unknown at this time.

| Admin inspt conducted as follow-up to inspection 2464737 for the reported release of brine and oil.Upon receipt of incident report, Dept called Opr rep Shane Kriebel via phone at 1507 and 1509 and left messages.Mr. Kriebel returned Dept’s phone calls at 1513.According to Mr. K, a brine tank failure resulted in a release of 5-10 bbl of brine water with some trace oil. The amount of oil is unable to be quantified. The release is believed to have resulted from a slit and/or hole in the bottom of the brine tank, but it is currently unknown what caused this failure to occur. Mr. Kriebel stated that the well-tender was on-site when the tank started to leak.Per Mr. K, the brine tank completely emptied and is therefore no longer leaking.The fluids are contained with booms and absorbent pads were utilized to soak up the trace amounts of oil.Excavation of the contaminated materials is scheduled to begin as soon as possible. | On-site inspection conducted in response to admin inspection ID 2464737 for the reported release of brine water and crude oil. Per Mr. McDaniels, a release of 13 bbl brine water and 1 bbl crude oil occurred as a result of the plastic brine tank catching fire at 1145 on 3/8/16. Mr. McDaniels stated that the release was contained to the well site and believes most of the fluids were burned off.

Mr. McDaniels stated that while the well-tender was blowing the well, the tank caught fire and exploded, and the lid landed approximately 50 feet off of the well site. No injuries were reported. At this time, the Operator is unsure as to what exactly caused the brine tank to ignite.Violations are being issued for this release of residual fluids reported by the Operator. Additional violations may be forthcoming upon further review and investigation.

On 3/8/16 at 1445, Operator representative Shane Kriebel reported a >5 gallon release of brine with trace oil to SWRO representative Martha Michalek. Per Mr. Kriebel, there was a 100 bbl plastic brine tank failure at the well site. The exact volume of the release is unknown at this time. The release is reported to have occurred and/or been discovered on 3/8/16 at 1230.

2464777 Primary Facility 2016-03-08 Administrative/File Review In Compliance with Policy Admin inspt conducted as follow-up to inspection 2464737 for the reported release of brine and oil.Upon receipt of incident report, Dept called Opr rep Shane Kriebel via phone at 1507 and 1509 and left messages.Mr. Kriebel returned Dept’s phone calls at 1513.According to Mr. K, a brine tank failure resulted in a release of 5-10 bbl of brine water with some trace oil. The amount of oil is unable to be quantified. The release is believed to have resulted from a slit and/or hole in the bottom of the brine tank, but it is currently unknown what caused this failure to occur. Mr. Kriebel stated that the well-tender was on-site when the tank started to leak.Per Mr. K, the brine tank completely emptied and is therefore no longer leaking.The fluids are contained with booms and absorbent pads were utilized to soak up the trace amounts of oil.Excavation of the contaminated materials is scheduled to begin as soon as possible. According to Mr. Kl, the release occurred today at 1230 and was subsequently reported to Dept a

2465416 Primary Facility 2016-03-09 Incident- Response to Accident or Event Violation(s) Noted On-site inspection conducted in response to admin inspection ID 2464737 for the reported release of brine water and crude oil. Per Mr. McDaniels, a release of 13 bbl brine water and 1 bbl crude oil occurred as a result of the plastic brine tank catching fire at 1145 on 3/8/16. Mr. McDaniels stated that the release was contained to the well site and believes most of the fluids were burned off.

Mr. McDaniels stated that while the well-tender was blowing the well, the tank caught fire and exploded, and the lid landed approximately 50 feet off of the well site. No injuries were reported. At this time, the Operator is unsure as to what exactly caused the brine tank to ignite.Violations are being issued for this release of residual fluids reported by the Operator. Additional violations may be forthcoming upon further review and investigation.

2535447 Primary Facility 2016-11-10 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to inspection record 2464737 (enf ID 340845) for the reported release of brine and oil.

Although a 15 day written response was received by the Department on 3/21/2016 notifying the Department that the Operator will be utilizing the Alternative Remediation method for the remediation of this release, a revised 15 day written response was requested by the Department on 3/22/2016 (inspection ID 2466803). This revised 15 day written response was received on 3/25/2016.

The report entitled “PA DEP Oil and Gas Alternate Process Combined Soil Site Characterization and Soil Remediation Action Completion Report” was received by the ECB program on 9/12/2016 and approved on 10/31/2016.

Operator appears to be in compliance with current policy at this time.

2598969 Primary Facility 2017-05-25 Routine/Complete Inspection No Violations Noted At time of inspection, well drilled and appears equipped for production. Well tag present, legible.

Department recommends operator remove any equipment not related to the production of the well from the site in accordance with current regulations.

Department observed a hissing noise possibly coming from the wellhead and also observed a moderate gas odor. This concern was immediately forwarded by phone to OGI Jason Branan for follow-up.
