2562718 Primary Facility 2017-02-16 Site Restoration No Violations Noted On 2/16/17 at 1510 hrs I conducted a restoration inspection following receipt of the Smith 606 Notice of Termination (NOT). It was partly cloudy and 31 degrees F. No one was present at the site.

Two well permits, both for the Smith 606 1V (117-20875 and 117-20915) were issued in 2010. The operator reported that the 1V well was not drilled and both permits have expired.

The NOT was received on 2/8/17 and states that the Smith 606 site has not been constructed. The NOT was reviewed on 2/16/17 and is administratively correct. A field review confirms that the site has not been built. I recommend approval of the NOT.

No violations were cited during this inspection. TLT
