| Inspection completed at the request of the opeartor to verify on-site that the well pad meets setback requirerments under 205(b) of the Oil and Gas Act.  Upon inspection I found the well stake with th permit # written on it. The site is within 100' ft of a small unmarked tributary to Elk Creek-however it is not a blue line stream on the topo therefore the operator does not need to apply for a wavier. Also there are a few small wet areas within 100' feet  of the well stake however the wet spots do not meet the wetland criteria. Operator was contacted and informed.

2020596 Primary Facility 2011-11-18 Routine/Partial Inspection No Violations Noted Inspection completed at the request of the opeartor to verify on-site that the well pad meets setback requirerments under 205(b) of the Oil and Gas Act. Upon inspection I found the well stake with th permit # written on it. The site is within 100' ft of a small unmarked tributary to Elk Creek-however it is not a blue line stream on the topo therefore the operator does not need to apply for a wavier. Also there are a few small wet areas within 100' feet of the well stake however the wet spots do not meet the wetland criteria. Operator was contacted and informed.

Upon inspection I found the BMPs remain in place- the access road remains in good condtion, the pits have been reclaimed, the site has been rough graded. Site is stable and awaiting good weather for final reclaim next spring.
