| Currently, well permit issued on 12/06/08 for well to be completed to 5,000 ft. in depth.  Well pad completed with filter fencing and one pit.  Well was completed and appeared to be hydro frac.  Well head contained 8¿ pipe reduced to 4¿ with 3 shut off valves.  Production equipment connected consisted of 100 bbl brine tank, heater and separator.  The well was non active during the site inspection.  No freeboard issues and drilling pit aligned with 20 mil plastic.  Water in the pit was in the process of transportation for disposal to Tunnel town ¿ Water treatment plant in Saltsburg.  Well sign and E&S plan located at the entrance way.  At the time of inspection, no E&S and IW issues at the site

Well pad completed with filter fencing and one pit. Well was completed and appeared to be hydro frac. Well head contained 8¿ pipe reduced to 4¿ with 3 shut off valves. Production equipment connected consisted of 100 bbl brine tank, heater and separator. The well was non active during the site inspection. No freeboard issues and drilling pit aligned with 20 mil plastic. Water in the pit was in the process of transportation for disposal to Tunnel town ¿ Water treatment plant in Saltsburg. Well sign and E&S plan located at the entrance way. At the time of inspection, no E&S and IW issues at the site
1884091 Primary Facility 2010-05-10 Site Restoration Violation(s) Noted At the time of inspection, the well was properly tagged and in production. Drilling was completed July 13, 2009. Sign and E&S plan onsite. It is recommended the plan be replaced, as it had been damaged by rain, until final restoration activities have been completed. Access road was stable. Brine tank appeared in sound condition. Several violations were observed, first a failure to properly restore site within 9 months after completion (07.13.09) and upon dept. receiving restoration report. It also appeared no efforts were taken to regrade or mulch and seed the area to permanently stabilize the site. Also observed a failure to implement bmp¿s to reduce sedimentation & erosion issues, I noted significant ponding of water in various locations on and surrounding site.
