On February 9, 2012, I stopped at the site and confirmed that work was halted.

| On February 10, 2012, WQSS Vince Yantko and I visited the Kurutz site to evaluate the situation.  The site was still shut down and an environmental contractor was monitoring for hydrogen sulfide.  This contractor reported that there were no new detects.

We talked briefly about the clean-up and walked around the open pit.  It appeared like more water was coming into pit and Vince expressed concerns that pit water inflow may reach a level where pit contents may reverse flow back out of liner hole.

Later that day, Vince received a tip from a citizen that fly ash was delivered to this well site to be used as a bulking / drying additive for pit preparation for landfill disposal. Chevron was notified by phone & requested to investigate & confirm all additives used.

| On February 14, 2012, I stopped at the Kurutz site to check progress.  They were still not working on site but the monitoring contractor was still looking for hydrogen sulfide; they still hadn’t any hits.
About 120 barrels of liquid were removed from the pit in the last two days.

Chevron was working on a plan to empty the pit and handle any hydrogen sulfide problems.

| On February 15, 2012, Chevron’s contractor again started removing material from the cuttings pit.

On February 17, 2012, I stopped to check the progress.  Material was actively being removed at inspection and I got to see them mix the cement with the cuttings.  The work was progressing and they had no more hydrogen sulfide hits at that point.

They hoped to have all the work including the liner removed by Wednesday, February 22nd.

Beth Smock from Chevron was on-site and we spoke briefly about the case.

| On 06/26/15 at approximately 1500 hours I conducted a routine complete inspection of the Kurutz Unit. The Act 9 sign was in place and visible. All required documents were present in the mailbox. The site was well vegetated and no signs of accelerated erosion were noted. The well heads appeared to be in good condition. There were no violations noted at the time of inspection.
