| On February, 8, 2012, while subcontractor was removing / mixing the material in the cuttings pit, a Chevron employee got a reading of 22 ppm hydrogen sulfide (H2S) on his PPM.  All work was halted and the site evacuated until the situation was resolved.

| On February 10, 2012, WQSS Vince Yantko and I visited the Kurutz site to evaluate the situation.  The site was still shut down and an environmental contractor was monitoring for hydrogen sulfide.  This contractor reported that there were no new detects.

We talked briefly about the clean-up and walked around the open pit.  It appeared like more water was coming into pit and Vince expressed concerns that pit water inflow may reach a level where pit contents may reverse flow back out of liner hole.
 
 
Later that day, Vince received a tip from a citizen that fly ash was delivered to this well site to be used as a bulking / drying additive for pit preparation for landfill disposal. Chevron was notified by phone & requested to investigate & confirm all additives used.

| On 06/26/15 at approximately 1500 hours I conducted a routine complete inspection of the Kurutz Unit. The Act 9 sign was in place and visible. All required documents were present in the mailbox. The site was well vegetated and no signs of accelerated erosion were noted. The well heads appeared to be in good condition. There were no violations noted at the time of inspection.

Bill’s investigation revealed that this pit does not comply with requirements under Chapter 78 – specifically 78.56 a 4 iii.

The Act 9 sign was in place and visible. All required documents were present in the mailbox. The site was well vegetated and no signs of accelerated erosion were noted. The well heads appeared to be in good condition. There were no violations noted at the time of inspection.
