| Inspection the result of the Operator reporting a breached BMP during a precipitation event.  The Operator is in compliance with the Departments notification policy for a BMP breach.  During the inspection the Department notes that an infiltration ditch located closest to the access road failed during a heavy precipitation event in the early morning hours on 6/13/2013.  The failure resulted in material from the infiltration ditch being washed into a 32" silt sox, which did not contain the material and also failed.  The two failures resulted is accelerated erosion of the access road and scouring of a hillside below the excavated pad area.  The precipitation event appears to exceed the Departments criteria for construction standards.  The Operators response was to rebuild the failed portion of the infiltration ditch, add an additional rock outlet and repair the silt sox.  The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

The Department observed sedimentation in the outlet rip rap on both sides of the road.  The second area is located within the LOD between the well pad and area reserved for the CI.  The Department suggest the operator add clean durable rock to the make shift basin and outlet up to the access road, and also clean the reinstall the silt sox to prevent sedimentation from leaving the LOD.  The Department also notes that hay bales are not ABACT approved in HQ watersheds.  The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

The Operator is in compliance with the Departments notification policy for a BMP breach. During the inspection the Department notes that an infiltration ditch located closest to the access road failed during a heavy precipitation event in the early morning hours on 6/13/2013. The failure resulted in material from the infiltration ditch being washed into a 32" silt sox, which did not contain the material and also failed. The two failures resulted is accelerated erosion of the access road and scouring of a hillside below the excavated pad area. The precipitation event appears to exceed the Departments criteria for construction standards. The Operators response was to rebuild the failed portion of the infiltration ditch, add an additional rock outlet and repair the silt sox. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.

The Department observed sedimentation in the outlet rip rap on both sides of the road. The second area is located within the LOD between the well pad and area reserved for the CI. The Department suggest the operator add clean durable rock to the make shift basin and outlet up to the access road, and also clean the reinstall the silt sox to prevent sedimentation from leaving the LOD. The Department also notes that hay bales are not ABACT approved in HQ watersheds. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
