Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2411072 Primary Facility 2015-09-24 Drilling/Alteration No Violations Noted 9/24/2015 Cellars in place.
2413681 Primary Facility 2015-09-24 Preoperation Inspection No Violations Noted Pre-Operation inspection in response to the Operators request received on 9/23/2105.
2426859 Primary Facility 2015-11-18 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was actively drilling the well. The drill rig was onsite and placed on containment. The E&S plan was onsite. No observation of accelerated erosion or sedimentation in relation to the well pad or access road. The perimeter controls and BMPs were properly installed and appeared to be functioning properly. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2448311 Primary Facility 2016-02-09 Routine/Partial Inspection No Violations Noted At the time of inspection the well was drilled and the Operator was actively working to install the production equipment.
2497577 Primary Facility 2016-06-28 Complaint Inspection No Violations Noted At the time of inspection the well was drilled and equipped for production. The well apparatus, brine tanks, and gas production units appeared to be in sound condition.

Inspection also in response to complaint 320656, the complaint indicates that “waste water from the site contains elevated LEL levels and routinely flashes and is being mixed with other wastes”. During the onsite inspection the brine tank appeared to be sound condition and no observation of onsite mixing was observed by the Department. This complaint is being forward to Waste Management for follow up at the waste disposal facility listed in the complaint.

2516371 Primary Facility 2016-07-20 Complaint Inspection Pending At the time of inspection the well was drilled and equipped for production. The site inspection is in response to a complaint #320656 filed with the Department on 6/28/2016. The complainant described possible hazardous conditions on site.

All of the production equipment is located on the right side of the well site, which is the cut slope. The slope is approximately 10’ in height, gradual and opens up into a large meadow. When the Department arrived on site WQS Steingrabe observed a vapor emanating from the top of a plastic tank connected to the dehydration unit. Combustible gas readings were taken by ERT staff near the production equipment, specifically the 800 gallon opaque plastic tank, which appeared to be attached to the dehydration unit. The combustible gas readings are summarized on the attached ERT inspection. The 800 gallon tank appeared to contain approximately 300 gallons of liquid. The lid on the plastic tank was no opened, however the plastic tank was opaque and the fluid contained in the a

2516375 Primary Facility 2016-07-28 Follow-up Inspection No Violations Noted Follow up inspection to inspection conducted on July 20, 2016, see inspection #xxxxxxx , this inspection is also related to Complaint # 320656.

On July 28, 2016 the Department requested an on site meeting with the Operator. The Operator agreed to and scheduled the meeting for July 29, 2016. The Operator sent two subcontractors, L. Casaday from Rock Run, and Jim Drinkwine from Danos. Also onsite during the inspection was PADEP WQS S. Steingrabe and PADEP OGI J. Brannan. The Department began by reviewing the complaint and the observations during the 7/20/2016 inspection. The Department informed Mr. Casady and Mr. Drinkwine that combustible gas measurements taken 7/20/2016, resulted in the detection of combustible gas at levels exceeding 100% LEL. The measurements were taken at the vent on top of the 800 gallon plastic tank located near the dehydration unit. The 800 gallon plastic tank is situated inside plastic secondary containment container. The fluid inside the secondary containment container appeared

2521158 Primary Facility 2016-09-21 Routine/Partial Inspection No Violations Noted Follow up inspection to inspection's #2497577, 2516371 and 2516375.

At the time of inspection the 800 gallon plastic appeared to be decommissioned. The 800 gallon plastic tank was disconnected from the dehydration unit and was empty. The smaller plastic tank, located inside the dehydration unit secondary containment, was also removed. No standing water observed inside the dehydration unit secondary containment.

2798930 Primary Facility 2018-11-06 Routine/Complete Inspection No Violations Noted At the time of inspection well equipped for production. Brine tank within secondary containment and appears structurally sound. GPU within secondary containment. No E & S issues.
