| I met Erick Duzick of PGE when I arrived.  Frac was active, but currently pressured down.  Containment was in good condition, with foam berms and stone piled against it on the outside.  In the pump truck area, water had collected in containment that showed slightly elevated conductance (1.93 mS/cm) and had a petroleum sheen on it.  Near the Sandstorms, clean sand had been piled on the ground and the berm, compressing the berm.  I also noted water that had collected in a Baker tank battery containment (approximately 20-40 gallons) that showed elevated conductance (5.2mS/cm); pad surface was dry immediately next to this area.  I informed Erick of all of these issues, and he stated that he would have the water vacced and the berm cleared of the sand.

I entered the site with Ben Gamble (DCNR) at approximately 11:00. After we entered the site we checked in with the company man (Alex Strittmatter) and then walked the site.  At the time of the inspection, they were on stage 19 of 37 on the COP Tract 322 Pad C 2675 (081-21272). The site containments were all in good condition and rig mats were installed in high traffic areas. Water accumulated in the produced water containment had field conductivity of 250 umhos/cm and had a sheen. Water accumulated on the pad surface had field conductivity between 930-1,000 umhos/cm. When we went back to the company man we recommended the water in containment (with a sheen) be vacuumed. We also recommended the operator swap out the open-top waste container/roll-off with a covered waste container. DCNR relayed recent experiences with waste blowing out of the container or being removed by wildlife while scavenging food.

| Incident inspection at PGE's COP Tract 322 Pad C with production supervisor Jody Weidner of PGE on May 7, 2018. According to Mr. Weidner, flowback from all 5 recently stimulated wells (081-21268, 081-21629, 081-21270, 081-21271, 081-21272) was flowing into (1) vertical 210 BBL steel produced fluids tank stationed in secondary containment. From there flowback water was being gravity fed via a 4" hose to the AST located onsite. At some point during the night of May 5 into the morning of May 6, manifolded valves from the steel produced fluids tank to the (3) vertical, poly tanks were switched on. This resulted in the 3 manifolded poly tanks to start filling up with flowback water. According to Mr. Weidner, one of the three poly produced fluids tank was missing a plug located on the upper-side of the tank. Once the flowback water reached this level in this poly produced fluids tank, water started spilling out of the hole. | Light activity on the site when I arrived.  MASS was still in place, with water lines running from the production tanks onsite to the tank; water lines running to the 322 E had been dismantled.  The Well Water Solutions MASS was approximately half full of wastewater, and there was approximately 3-5" of what appeared to be rainwater in the secondary containment MASS.  Areas around the MASS, including the truck offloading area, were dry and clean.  Work was occurring at the wellheads; when I checked the wells, I was initially unable to find the permit numbers posted on them.  When I was able to locate the tag, it was very lightly embossed in the well steel itself, and was very difficult to read; I recommend that the permit numbers be installed on the wells in a more legible fashion.  An undetermined amount of water was present in the well cellars.  I recommend that the water be vacced from both the cellars and the secondary containment MASS.  I met Jody Weidner of PGE as I was leaving, and mentioned the well t

This plug is located near the open/close handle on the discharge hose (see picture 5). | I arrived onsite in response to a spill report for the site.  Ricky Ristau of PGE called at ~9:45 on July 2nd to report that at ~7:30 AM, a water truck filling from the AST overtopped and spilled over 5 gallons of produced water to the pad surface.  Will Smith and Clayton Schucker of Moody & Associates were on location, and a small excavator and skid steer were excavating an area ~25' x 15' to the east of the AST.  In the excavated area, conductivity readings in the soil were low (.05-.25 mS/cm); readings outside the excavation were mostly in a similar range.  I did find that the area being excavated needed to be expanded ~1-2' further from the pad, as soil conductivity readings in that area were elevated (4.00 mS/cm, the upper limit if my meter).  Will and Clayton also had soil probes, and used white flags to delineate the area to be excavated.  Jodi Weidner of PGE arrived at this point, and I explained to him that the excavation area had been expanded.  We spoke about the spill, and Jodi told me that he wa

2699553 Primary Facility 2018-02-28 Routine/Complete Inspection No Violations Noted I met Erick Duzick of PGE when I arrived. Frac was active, but currently pressured down. Containment was in good condition, with foam berms and stone piled against it on the outside. In the pump truck area, water had collected in containment that showed slightly elevated conductance (1.93 mS/cm) and had a petroleum sheen on it. Near the Sandstorms, clean sand had been piled on the ground and the berm, compressing the berm. I also noted water that had collected in a Baker tank battery containment (approximately 20-40 gallons) that showed elevated conductance (5.2mS/cm); pad surface was dry immediately next to this area. I informed Erick of all of these issues, and he stated that he would have the water vacced and the berm cleared of the sand.
2700908 Primary Facility 2018-02-21 Routine/Complete Inspection No Violations Noted Routine inspection, 2/21/2018.

I entered the site with Ben Gamble (DCNR) at approximately 11:00. After we entered the site we checked in with the company man (Alex Strittmatter) and then walked the site. At the time of the inspection, they were on stage 19 of 37 on the COP Tract 322 Pad C 2675 (081-21272). The site containments were all in good condition and rig mats were installed in high traffic areas. Water accumulated in the produced water containment had field conductivity of 250 umhos/cm and had a sheen. Water accumulated on the pad surface had field conductivity between 930-1,000 umhos/cm. When we went back to the company man we recommended the water in containment (with a sheen) be vacuumed. We also recommended the operator swap out the open-top waste container/roll-off with a covered waste container. DCNR relayed recent experiences with waste blowing out of the container or being removed by wildlife while scavenging food. 2728654 Primary Facility 2018-05-07 Incident- Response to Accident or Event Violation(s) Noted Incident inspection at PGE's COP Tract 322 Pad C with production supervisor Jody Weidner of PGE on May 7, 2018. According to Mr. Weidner, flowback from all 5 recently stimulated wells (081-21268, 081-21629, 081-21270, 081-21271, 081-21272) was flowing into (1) vertical 210 BBL steel produced fluids tank stationed in secondary containment. From there flowback water was being gravity fed via a 4" hose to the AST located onsite. At some point during the night of May 5 into the morning of May 6, manifolded valves from the steel produced fluids tank to the (3) vertical, poly tanks were switched on. This resulted in the 3 manifolded poly tanks to start filling up with flowback water. According to Mr. Weidner, one of the three poly produced fluids tank was missing a plug located on the upper-side of the tank. Once the flowback water reached this level in this poly produced fluids tank, water started spilling out of the hole. Once the operator observed this and the accumulated flowback water within the containment,
2738441 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted Light activity on the site when I arrived. MASS was still in place, with water lines running from the production tanks onsite to the tank; water lines running to the 322 E had been dismantled. The Well Water Solutions MASS was approximately half full of wastewater, and there was approximately 3-5" of what appeared to be rainwater in the secondary containment MASS. Areas around the MASS, including the truck offloading area, were dry and clean. Work was occurring at the wellheads; when I checked the wells, I was initially unable to find the permit numbers posted on them. When I was able to locate the tag, it was very lightly embossed in the well steel itself, and was very difficult to read; I recommend that the permit numbers be installed on the wells in a more legible fashion. An undetermined amount of water was present in the well cellars. I recommend that the water be vacced from both the cellars and the secondary containment MASS. 2739844 Primary Facility 2018-06-07 Incident- Response to Accident or Event Violation(s) Noted Department representative Sue Herzing conducted an incident inspection at PGE's COP Tract 322 Pad C with production supervisor Jody Weidner of PGE on June 7, 2018. Nathan Harris of PGE reported a small spill of 5-10 gallons of brine near the AST (Above-ground Storage Tank) earlier on the morning on June 7, 2018.

When I arrived onsite, the operator had a small excavator onsite removing impacted soils near the AST. The excavated soil was being placed on plastic during my inspection. Mr. Weidner was using a soil conductivity probe to delineate impacted soils.

According to Mr. Weidner the release occurred during the process of a brine truck receiving fluids from the AST. A plug was knocked out of the bleeder valve prior to the truck driver disconnecting. This plug is located near the open/close handle on the discharge hose (see picture 5). This open bleeder valve resulted in fluids from the AST being released onto the secondary containment and outside of the secondary containment. Another truck driver found

2748716 Primary Facility 2018-07-03 Incident- Response to Accident or Event Violation(s) Noted I arrived onsite in response to a spill report for the site. Ricky Ristau of PGE called at ~9:45 on July 2nd to report that at ~7:30 AM, a water truck filling from the AST overtopped and spilled over 5 gallons of produced water to the pad surface. Will Smith and Clayton Schucker of Moody & Associates were on location, and a small excavator and skid steer were excavating an area ~25' x 15' to the east of the AST. In the excavated area, conductivity readings in the soil were low (.05-.25 mS/cm); readings outside the excavation were mostly in a similar range. I did find that the area being excavated needed to be expanded ~1-2' further from the pad, as soil conductivity readings in that area were elevated (4.00 mS/cm, the upper limit if my meter). Will and Clayton also had soil probes, and used white flags to delineate the area to be excavated. 2822567 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted On January 3, 2019, Department representative Brad Kline conducted a routine site inspection at PGE Company LLC. DCNR Tract 322 Pad C. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover. No well pad restoration has taken place.

There are a total of 5 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 5 line heaters and each well had a sand separator associated with it. 1 reboiler unit was on the pad. 1 steel and 5 poly vertical production fluids tanks onsite. Both line heaters and production fluid tanks were stationed in secondary containment.
