2398981 Primary Facility 2015-08-10 Routine/Complete Inspection No Violations Noted Access shared with ag field. At the time of inspection well appears equipped for production. Well tag present and affixed in a permanent manner. Site permanently stabilized. No E&S issues in relation to site and/or access road. Brine tank located on site and appears to be structurally compromised. A slow leak from the flange connection to the brine tank discharge outlet and a small damp area beneath the discharge valve was observed. No immediate impacts of dead and/or distressed vegetation observed as a result of this compromise. Department spoke with Ms. Kristen (Operator) and informed of above findings. Recommended Operator follow-up with Department upon investigation of reported concerns and to quantify release and identify all materials released if applicable within 24 hours.
