| On 2/4/19 at 1414 hrs., I conducted a routine inspection of MDS Energy Development, LLC's Gilbert C Querio 1A-43 well (API 005-30250) in Gilpin Twp., Armstrong Co.  The well pad is restored and permanently stabilized.  The well is equipped for production.  A site ID sign is present at the entrance to the well pad.  A well tag is affixed to the well head. There are no violations noted for this inspection.
