Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2194759 Primary Facility 2013-06-27 Routine/Complete Inspection No Violations Noted Someone has been digging around the well. Checked for methane w/ MSA meter, 0% methane and 0 on comb.

2377006 Primary Facility 2015-06-03 Plugging(Includes Plugged/Mined Through) No Violations Noted Met SWN staff Dave Wilcox, Mike Tornes and land liaison Wally @ 8:00 and drove to the Comiskey #1 well (plugged by Shell in 1973). SWN is investigating the well due to its proximity of some of its other wells. The well is in a vegetated field. Away from the well head, small ponds and wetlands were visible. Hanover staff were conducting a resource evaluation at the time of my inspection. The well was buried, but a vent pipe with ball valve and threaded plug was shut in. When this valve was opened, I verified that combustible gas was present in this vent using an Altair 5X Multigas meter. I left the location at 8:30 and returned at 11:00. By that time excavation had removed wood boards (a wood cellar?) and revealed the 16”, 11-3/4”, and 8-5/8” casings. We observed bubbling in the 11-3/4” x 8-5/8” annulus only (see photo). The vent pipe emerged from this annulus and gas was emerging from the base of the vent.
