| The site is utilized as a water processing facility for flowback operations.  Flowback material goes through a filtering process for separation of solids of liquids.  Flowback water is pumped into a centralized impoundment for storage and then pumped into a another centralized impoundment blended with fresh water.  Water is pumped through underground lines to nearby well pads to be used for fracing operations.  During the inspection DeSoto Rig #9 was in the process of drilling Greenweig #3H, Permit number 37-015-20422.  Several small tears were observed throughtout the course of the containment liner.  For future operations it is recommended to have a larger buffer zone in the solids handling area and chemical storage area.  Both areas were directly next to fill slope and berm of well pad, with minimal room for spilled material to remain on containment and potentially the well pad.  Also it is recommended to improve housekeeping and patching of the containment liner at the site.

| The site is utilized as a water processing facility for flowback operations.  Flowback material goes through a filtering process for separation of solids of liquids.  Flowback water is pumped into a centralized impoundment for storage and then pumped into a another centralized impoundment blended with fresh water.  Water is pumped through underground lines to nearby well pads to be used for fracing operations.  During the inspection DeSoto Rig #9 was in the process of drilling Greenweig #3H, Permit number 015-20422.  Several small tears were observed throughtout the course of the containment liner.  For future operations it is recommended to have a larger buffer zone in the solids handling area and chemical storage area.  Both areas were directly next to fill slope and berm of well pad, with minimal room for spilled material to remain on containment and potentially the well pad.  Also it is recommended to improve housekeeping and patching of the containment liner at the site.

| On June 6, 2017, at 1200 hrs., the Department conducted a routine inspection at the SWN Production Company LLC, GU-C-Pad in Herrick Township, Bradford County.  Andrew Hobbins, Water Quality Specialist, represented the Department.  A SWN representative was on site performing maintenance work on the adjacent waste impoundments.

| The DEP conducted a RESTORATION inspection of SWN's Greenzweig (C Pad) in Herrick Twp, Bradford County on 12/18/2018 at 1155 hours. The inspection was conducted in response to a Well Site Restoration Report received by the DEP.

The site has recently been disturbed to install PCSM BMPs per restoration plans. The areas that were disturbed now have straw/hay layered to assist in vegetation growth in the Spring. The areas are poorly vegetated now and are not stabilized. Silt sock was observed beneath the newly constructed Bioretention basin. This site does not meet the requirements for restoration under a well site restoration report. The site is not stabilized with 70% vegetative cover and temporary E&S BMPs are still installed. It is understood that Southwestern has a Consent & Order agreement with the department to install these newly permitted PCSM BMPs. Upon further discussion, the sites already inspected will have to be returned.

During the inspection, I used an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen the road side seep, the seep area outside the tank farm on the slope, the rock-lined swale, and underdrain pipe below the samller impoundment.  Conductivity readings in the road side seep indicated a high reading of 12 milliSiemens/cm; 9 milliSiemens/cm inside the tank farm, 10 milliSiemens/cm on the slope outside the containmant area, 9 milliSiemens/ cm in the rock-lined swale and 10 millSiemens/cm in the underdrain below the impoundment.

Flowback material goes through a filtering process for separation of solids of liquids. Flowback water is pumped into a centralized impoundment for storage and then pumped into a another centralized impoundment blended with fresh water. Water is pumped through underground lines to nearby well pads to be used for fracing operations. During the inspection DeSoto Rig #9 was in the process of drilling Greenweig #3H, Permit number 015-20422. Several small tears were observed throughtout the course of the containment liner. For future operations it is recommended to have a larger buffer zone in the solids handling area and chemical storage area. Both areas were directly next to fill slope and berm of well pad, with minimal room for spilled material to remain on containment and potentially the well pad. Also it is recommended to improve housekeeping and patching of the containment liner at the site.

2072780 Primary Facility 2012-06-04 Administrative/File Review Violation(s) Noted Upon review of the groundwater sample results, it was noted that the C Pad-Sump Drain Below Tank Farm had elevated concentrations of barium, iron, manganese, chloride and bromide. Barium was detected above the Act 2, Residential and Non-residential Medium Specific Concentration (MSC) for groundwater of 2,000 micrograms per liter (ug/l) with a concentration of 2,310 ug/l. Iron was detected above the Act 2 Secondary Maximum Contaminant Level (SMCL) of 300 ug/l with a concentration of 5,980 ug/l. Manganese was detected above the Act 2 SMCL of 50 ug/l with a concentration of 2,390 ug/l. Chloride was detected above the Act 2 SMCL of 250,000 ug/l with a concentration of 1,190,000 ug/l. Bromide was detected at a concentration of 9,850 ug/l.
2074892 Primary Facility 2012-06-11 Administrative/File Review Violation(s) Noted Upon review of the groundwater sampling results, it was noted that groundwater monitoring well MW-4 exhibited elevated concentrations of chloride ranging from 160,000 micrograms per liter (ug/l) to 234,000 ug/l. In addition, the groundwater data collected prior to operations and introduction of water in the impoundments exhibited elevated concentrations of 210,000 ug/l from a sample collected on 7/16/2010 and 200,000 ug/l from a sample collected on 8/4/2010. Drilling operations at the Greenzweig 1V occurred from 3/8/2008 to 3/24/2008 and at the Greenzweig 1H drilling operations occurred from 9/20/2008 to 10/19/2008. Based on the elevated chloride detected in the groundwater samples and the drilling operations pre-dating the groundwater monitoring, site characterization activities are warranted at the C-Pad.
2074984 Primary Facility 2012-05-15 Incident- Response to Accident or Event Pending On May 11, 2012 Southwestern and the Department held a conference call in reference to samples collected at the C Pad site after issues with the tank farm were suspected and stressed vegetation was observed at the rock lined swale location down gradient of the tank farm and parallel to the 40,000 barrel capacity centralized impoundment, otherwise termed the smalller impoundment.
2098001 Primary Facility 2012-09-05 Administrative/File Review Violation(s) Noted On 8/27/2012, the Department collected a sample of the waste material at the C-Pad. The collection of the sample was in response to the elevated concentration of chloride detected in monitoring well MW-4 and the subsequent Notice of Violation dated June 12, 2012. Southwestern’s groundwater monitoring well MW-4 exhibited elevated concentrations of chloride ranging from 160,000 micrograms per liter (ug/l) to 234,000 ug/l. On 8/27/2012, Southwestern mobilized to the C- Pad to implement the work plan. During the installation of test pit (1) and (2) dark gray material was encountered at depths ranging from approximately (6) to (8) feet below ground surface (bgs). At test pit location (2) a sample of the dark gray material was collected by the Department and submitted for laboratory analysis. Synthetic plastic liner was not observed at the locations of test pit (1) and (2). Upon review of the sample results, it was noted that the C-Pad test pit location (2) sample had elevated concentrations of arsenic, barium, bromide, chloride and iron.

2603352 Primary Facility 2017-06-06 Routine/Complete Inspection No Violations Noted On June 6, 2017, at 1200 hrs., the Department conducted a routine inspection at the SWN Production Company LLC, GU-C-Pad in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A SWN representative was on site performing maintenance work on the adjacent waste impoundments.

The site sign, E & S plan and the weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. PCSM has not been addressed at the site, but the site has been re-permitted and upgraded to an ESCGP-2. PCSM installation is scheduled for the month of June.

Seven wells were in production phase during the inspection. All of the wells were labeled and no bubbling was observed in the well cellars. Two production tanks and seven GPUs were also observed on site. The production tanks were observed in secondary containment. Puddles on the pad surface were low in specific conductivity. 2818305 Primary Facility 2018-12-18 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Greenzweig (C Pad) in Herrick Twp, Bradford County on 12/18/2018 at 1155 hours. The inspection was conducted in response to a Well Site Restoration Report received by the DEP.

The site has recently been disturbed to install PCSM BMPs per restoration plans. The areas that were disturbed now have straw/hay layered to assist in vegetation growth in the Spring. The areas are poorly vegetated now and are not stabilized. Silt sock was observed beneath the newly constructed Bioretention basin. This site does not meet the requirements for restoration under a well site restoration report. The site is not stabilized with 70% vegetative cover and temporary E&S BMPs are still installed. It is understood that Southwestern has a Consent & Order agreement with the department to install these newly permitted PCSM BMPs. Upon further discussion, the sites already inspected will have to be returned.

This Well Site Restoration Report is considered returned at this time.
