Currently, 16" conducter set at 44 ft., 113/4" pipe set at 465 ft., 8 5/8" set at 1200 ft.  Cement - 275 sks for 11 3/4", 8 5/8" 305 sks. Each cement had return BBl of 1 and 4 respectively.  Coal seems observed at 422 to 427 ft.  According to Driller, no open mine.  Soft zone but drill cuttings were observed through this zone.  Complete well log was on-site.

Dept recently received a request for an extension on restoration for an additional six months, which is currently being reviewed. No violations noted during at the time of this inspection.

Observed one structurally sound, lined below grade pit on well site. Impoundment was unlined/unfilled at the time of inspection. Dept recently received a request for an extension on restoration for an additional six months, which is currently being reviewed. No violations noted during at the time of this inspection.
1908106 Primary Facility 2010-08-17 Routine/Complete Inspection No Violations Noted site was properly signed at entrance with E&S plan available. Well drilled, not yet fraced.. Accompanied onsite by WQS Curtis Small, MRPS Jim Kline, T&F Rep Ken Lockard, and Engineer Mike Beeghely. Impoundment was currently filled with 600,000gals freshwater. Operator had plans to fill to 1 million gals and then fence the impoundment, which appeared structurally sound. Filter fence was properly installed and functioning down gradient of freshwater impoundment, recommended filter fence be extending along side of impoundment furthest from access road to prevent the potential for accelerated E&S concerns. Again, no E&S concerns were noted on site or access road. Onsite pit was lined/empty. Brush grub and filter fence were installed below gradient and properly functioning. Access road adequately rocked Dept previously denied operator request for extension, BUT TODAY dept received additional documentation of validation for the request from fracing co; therefore request is being re-reviewed. 1912980 Primary Facility 2010-09-01 Routine/Complete Inspection No Violations Noted During this inspection, well is being actively fraced by Superior Wells. Site properly signed with updated E&S available for review. Operator also has security at entrance to well access road. No accelerated erosion or sedimentation issues were observed on well site, pad, or impoundment. All pits sustained the required 2ft of lined freeboard. Recommendations were made to operator to reposition flow back line near pit in order to prevent a possible pollution event and/or excision of pit freeboard. Flow back was not occurring during this inspection. Impoundment appears structurally sound. Observed that operator placed orange security fence surrounding the perimeter of impoundment.
1915995 Primary Facility 2010-09-28 Plugging(Includes Plugged/Mined Through) No Violations Noted T&F exploration plugging well. As outlined in the plugging procedure, bridge plug set at 8500 ft. Pipe cut at 7500 ft or 62 joints. First plug of cement set to 5130 ft. At the time of inspection, both pits were with plastic and no freeboard issues. Plastic was in acceptable condition. Well sign and E&S plan located at entrance way. Additional pipe was in the process of pulling to set second plug. At the time of inspection, no E&S or IW issues. Met representatives at the site form T&F. Permit was on-site.
1920296 Primary Facility 2010-10-12 Complaint Inspection No Violations Noted Well is permanently plugged with erected monument in place. Sub-contractor onsite actively reclaiming (regarding) well pad, drill pit had been removed. Impoundment pit is empty and intact. No accelerated erosion or sedimentation issues noted in relation to access road or well site. No IW issues observed. Spoke via telephone with Ken Lockard of T&F whom stated that drill pit was pumped out by Dively¿s Trucking and transported to impoundment prior to final disposal. Dept requested manifest and/or written narrative describing how ALL pits on and off site were used throughout the fracing and plugging process as well as how materials were disposed of and ultimately how the pit was removed/reclaimed. Spoke to sub contractor onsite, about how pit was removed; unfortunately another crew was onsite last weekend when the first measures of site restoration began. Dept currently waiting on operator response. Based on site inspection, no violation noted.
1920309 Primary Facility 2010-10-13 Complaint Inspection No Violations Noted During this inspection, site signed with ES plan. Well is permanently plugged with erected monument in place. Sub-contractor onsite actively reclaiming (regarding) well pad, onsite drill pit had been removed. Impoundment pit is empty and intact. No accelerated erosion or sedimentation issues noted in relation to access road or well site. No IW issues observed. Spoke via telephone with Ken Lockard of T&F whom stated that drill pit contents were pumped out and transported by Dively¿s Trucking, and then pit liner was folded over top of pit and subsequently backfilled on 10/8/10 around midday. On 10/7/10, Ken stated that to his observation all liquid fraction had been removed from drill pit, pit in question. Dept is currently awaiting response from operator with pit use and disposal methods, as well as manifests. During, this site inspection, no violations were noted at this time BUT final results pending on pit disposal methods recieved from operator.
1920321 Primary Facility 2010-10-14 Complaint Inspection Pending Admin inspt -Spoke with Bill Straslicka and Ken Lockard of T&F Exploration. Bill confirmed Ken¿s recollection of events, that pit was pumped out by Dively trucking and that prior to back filling (to their knowledge) all liquid fraction had been removed from drill pit, pit in question by complainant. Ken noted that overnight on Thurs Oct 7, 2010, heavy rains occurred in the area and pit was backfilled on Friday Oct 7, 2010, which may have been what was seen in the pit (if indeed liquid was noted) earlier in the day prior to backfilling., but last observation no know liquid was in drill pit. The drill pit was lasted used for the plugging operation for in/out flow. Impoundment pit is empty of all contents and scheduled to be backfilled/restored either tomorrow or Monday (10/17/10) weather pending. Operator agreed to provide dept with written correspondence on pit contents/disposal methods and manifests/narratives from hauler. Based on current findings it does not appear that liquid flowback was backfilled, BUT FINAL RESULTS ARE P E N D I N G
1932199 Primary Facility 2010-12-02 Follow-up Inspection Violation(s) Noted Administrative Review ¿ The department recently requested operator to establish a detailed timeline and explanation on activities that occurred on the well site during Oct 4, specifically those related to the drill pit on site due to a complaint filed with the dept that ¿operator buried flowback.¿. Based on the response received by Department from T&F exploration dated 11/24/10, the dept concludes that the operator improperly encapsulated the drill pit on site by ¿puncturing liner¿ to disperse what operator estimates as 2,000bls of rainwater. Previously, a T&F rep stated to his knowledge that the fluids were pumped out of the pit prior to encapsulation/backfilling, but based on current information it appears that operator was not present during the encapsulation process and (based on ¿contractor¿s best recollection¿) that just prior to backfilling, the drill pit did contain fluids, about half full and contractor (Hillsdale) removed water ¿by puncturing liner at base with hoe.¿ Violations being issued for failure to comply with 78.60 abd 78.62.

1946293 Primary Facility 2011-01-25 Compliance Evaluation Pending At the time of inspection, well site properly signed with ES plan. Well drilled, completed (11/03/09) and subsequently plugged (09/2010). This inspection is a follow up to a violation issued on 12/02/10 regarding improper pit disposal. Dept requested that T&F conducted soil sampling following a dept pre-approved sampling plan. Chambers Environmental and T&F Reps Ken Lockard and Bill Straslicka were onsite to conduct soil borings and collect soil samples as outlined in sampling planning with a total of 32 soil samples. No pits or impoundments on site. Site and access road snow covered. No accelerated erosion or sedimentation issues observed. Once analysis is complete, department will review results and accordingly discuss further actions.
1959518 Primary Facility 2011-03-18 Routine/Complete Inspection No Violations Noted At the time of inspection, well ID and ES plan present. Well site locked. Meet Ken Lockard of T&F onsite. Access road ok, noted minor ES concerns due plowing of snows, operator spoke to Hillsdale Contracting about additional maintenance on access road and site to ensure permanently stability. Impoundment has been removed, area seeded/mulched with minimal growth. Site, all pits removed and disturbed areas regarded, seeded/mulched, minimal growth observed, operator has plans to reseed/ mulch as well as bring a roller to ensure adequate compaction of soil. Well plugged. Erected monument with permit Id attached. Operator has plans to remove access road from well site (per agreement with landowner). Currently awaiting receipt of pit soil sampling findings which will be reviewed by department and a final determination made. No accelerated ES issues note, bmp's in place until final restoration. Today, no violations observed. Follow up inspection will be conducted to ensure permanent restoration achieved. Accompanied by Ed Mokos during inspection.
2005521 Primary Facility 2011-06-14 Compliance Evaluation No Violations Noted ADMIN INSP- This inspection is being conducted as a result of a violation issued on 12/02/10 for improper pit encapsulation. Upon the department¿s request on12/09/10, T&F Exploration agreed to conduct a soil boring investigation to determine IF their improper encapsulation techniques may have adversely impacted (leached into) the ground beneath the former pit location. On June 14, 2011, the department collaboratively reviewed the final soil boring investigation results, submitted by Chambers Environmental on behalf of T &F Exploration, and concluded that based on the evidence provided from the soil analysis it does not appear (based on these findings) that former pit contents (cuttings, frac, or flowback) were released into the ground, although operator is still being held responsible for improper encapsulation techniques of pits.
2005528 Primary Facility 2011-09-16 Site Restoration No Violations Noted Well plugged with erected monument and API number. Noted minor erosion on access road mainly along broad based dips; therefore, recommend operator add additional rock to access roadway in order to prevent a possible accelerated erosion or sedimentation issue. Well site appears restored with vegetative cover and rock; both of which appear stable and adequate controls to sustain permanent stabilization capable of resisting erosion and sedimentation. Area previously disturbed by impoundment is successfully restored with >70% vegetative cover capable resisting erosion; therefore, department recommends temporary BMPs be removed. During this inspection no violations noted and well site appears restored.
