| At the time of inspection, Sidewinder 62 was being rigged up.  The conductor for the well had previously been set and plugged with pea gravel.  It was decided that the spud date would be the day that they started removing the pea gravel from the conductor.

Some of the stone remains in the pipe and will have to be removed by the rig.  Sound walls are up around 3/4 of the pad.  Only the front side is open.

| At the time of inspection, Sidewinder 62 was drilling 8 3/4" production hole.  BOP had been tested to 5000 psi on 3/11 and passed.  No oil, gas, or water has been encountered to this point.  Surface casing was cemented using 480 sxs (105 bbls) Class A, 15.6 ppg, 1.23 yield.  Displacement was 76 bbls freshwater and landed plug, floats held.  30 bbls cement returned to surface.

Intermediate casing was cemented using a lead of 155 sxs (35 bbls) Class A, 15.2 ppg, 1.26 yield and tail of 385 sxs (84 bbls) Class A, 15.6 ppg, 1.22 yield.  Displacement was 121 bbls freshwater and landed plug, floats held.  20 bbls cement returned to surface

| At the time of inspection, Sidewinder 62 has skid over to the #4H well.  The 2H had some tools left in well.  Company man believes the BHA couldn't handle the stress of handling too tight of a turn and they got hung up.  The lateral had to be shortened and tools were cemented in.  Casing was cemented using a lead of 695 sxs Class A (182 bbls), 4% bentonite, 14.4 ppg, 1.47 yield and tail of 970 sxs Class A (336 bbls), 50% Acid Soluble, 14.8 ppg, 1.94 yield.  Displacement was 173 bbls water.  Plug was bumped and floats held.  80 bbls spacer returned to surface so top of cement is estimated to be at 300'.

| On 1/24/2019, WQS Brian MacQuarrie onsite. The well site consists of three wells in production; 2H (019-22293), 4H (019-22294) and 6H (019-22295). The most recent well drilled was the 4H on 4/3/2018. The site has been stabilized with vegetative growth and Erosion and Sedimentation BMPs are maintained. The operator submitted a Well Site Restoration Extension request on 9/7/2018. Therefore, the well site restoration request is approved until 9/6/2020.

| At the time of inspection, Sidewinder 62 was being rigged up.  The conductor for the well had previously been set and plugged with pea gravel.  It was decided that the spud date would be the day that they started removing the pea gravel from the conductor.
 +

 +
