On the morning of June 26, 2018 at approximately 0820 a tanker truck was onsite removing water from the 14H 400 bbl produced fluids tank. While the tanker truck was dewatering from the 14H produced fluids tank, the level sensor ball within the truck tank malfunctioned, resulting in produced fluids overflowing from a 4" overflow hose located on the truck tank. Produced fluids spilled on the well pad via the overflow line for a few minutes before the truck driver was aware of the spill and could shut off the flow valve. While the truck driver was removing production water from the 14H produced fluids tank, the lease operator was simultaneously discharging water from within the produced fluids containment onto the gravel pad where the spill occurred.
