| A routine inspection was conducted at the Simonetti pad on Mar. 20, 2012. Three wells were drilled (3H, 4H, 5H). AMEC conducted an inspection on behalf of Ultra on Mar. 7, 2012. Erosion control best management practices were installed and functioning. The cut slope surrounding the pad is sparsely vegetated and there is evidence of accelerated erosion resulting in the forming of rills and sediment being transported to the rock-lined ditches. The cut slope surrounding the pad is sparsely vegetated and there is evidence of accelerated erosion resulting in the forming of rills and sediment being transported to the rock-lined ditches. The weather was 45°, cloudy, 72% humidity winds SW 2 mph. An adequate site ID sing was located at the entrance to the access road. Copies of well permits were visible on the backside of the site ID sign. A copy of the E&S plan along with the onsite inspection log was located in the mailbox. The site was last inspected by company personnel on 3/9/2016 with “no visible issues” listed in the comments section. |

The site consists of the access road and well pad. Three wells, the 3H - 5H, have been drilled and were placed on inactive status on 3/14/12. According to a restoration schedule provided by SWEPI to the Department, the Simonetti 817 site was restored in May 2016. The pad area has been minimized slightly and post construction stormwater management (PCSM) controls have been installed. Slopes surrounding the well pad are permanently stabilized as per 25 Pa Code Section 102.22(a). Unused areas of the pad are becoming stabilized with vegetation. A Well Site Restoration Report should be submitted as per 25 Pa Code Section 78a.65(e) and the Notice of Termination can be submitted as per 25 Pa Code Section 102.7. The last routine E&S inspection was conducted on 6/1/17. No violations are cited during this inspection. I concluded the inspection at 1224 |

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR). The site consists of the access road and well pad. Three wells, the 3H - 5H, have been drilled and were placed on inactive status on 3/14/12. The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed. Temporary E&S controls have been removed and the project is permanently stabilized as per 25 Pa Code Section 102.22(a)(2). The last routine E&S inspection was conducted on 6/15/17. Present onsite were myself and Dean Albright, OSR Shell. The weather was 40 degrees, rain. The pad surface was very muddy.
Upon arrival at the site, the recovered mud was being stored in the gas buster. Soil impacted by the release was in the front end loader bucket. A visible black staining and sheen was observed on the pad surface adjacent to the cellar. Mr. Albright stated Shell was in the process of plugging the 5H well. Freshwater was being pumped into the well bore to "gel the hole", when the oil based drilling mud was released from the B-annulus vent that had been accidentally left open. Mr. Albright hypothesized the mud was left over from when Ultra Resources originally drilled the well. He stated from the quantity of material it was possible Ultra was using the well |
