On March 13, 2013, a piece of equipment in Carrizo's YaharaSavage 1H well in Washington Township, Wyoming County, Pennsylvania broke during the final stages of drilling, releasing more than 200,000 gallons of flowback. Three families were evacuated due to concern of combustible natural gas build-up in the area, and were able to return the next day. Carrizo provided fresh water for the area residents until tests had been conducted showing that their water was still safe to drink. Residents were pleased with how Carrizo handled the situation. Contamination from the spill was mostly contained, though some got into shallow groundwater. Because of the size of the spill and the groundwater contamination, in addition to a later 9,000-gallon spill in April 2013, Carrizo was fined $192,044 by the DEP in June 2014.

| Conducted an inspection of the additional containment measures that were implemented to control fluids.  Fracking with other than freshwater had not commenced until the conclusion of this inspection.  Additional measures implemented at the site appear to have the potential to thoroughly address any further incident should it occur.  Some of the measures that were improved upon were the utilization of seperate manifolds and gauging for freshwater and produced water, backflow preventers and capability for lockout of produced water had been installed,  and tertiary containment was implemented in areas where the greatest potential for release exists.
