| Falcon Rig# 18 is the active drilling rig on the location.  Laddie Pendergraft is the company man on location.  The current drilling depth is (4,740) feet.  I observed a relatively large hole in the composite mats surveying the drilling rig containment system.  Beneath the composite mats are layers of felt and a (40) mil HDPE liner.  Mr. Pendergraft said the hole will be repaired today.  The secondary containment as part of the drilling rig and associated equipment appeared to be in good condition as well as did the production equipment secondary containment.  The fluid levels in the drill cuttings containers and Keystone Landfill containers did not exceed the (2)’ freeboard requirement of Title 25, Env. Protection, Pa. code 78.56.

| Arrived at the site at 12:00 in response to reported spills of (3-5) barrels of flowback water and (50) gallons of antifreeze.  Hydraulic fracturing operations were in progress.  Keane Frac is the subcontractor conducting well stimulation (frac) procedures on Innes South 8H well.  Justin Phillips, Health, Safety and Environmental Staff Coordinator Supervisor of SWN reported the estimated (3-5) barrel capacity spill flowback water spill that occurred on containment to the Department at 9:30 PM on 12/13/14.  Chris Becker, Health, Safety & Environmental staff Coordinator of SWN reported the estimated (50) gallon antifreeze spill to the Department on 12/14/14 at 2:30 PM.  There is no evidence the spills escaped secondary containment, no evidence of comprised liner integrity and no evidence of an impact to the pad surface in the general area where the spills occurred.  Both spills occurred due to mechanical malfunction according to SWN.

|  At time of inspection, arrived on site at 11:10a.m., the cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 587psi on the production casing and 508psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:05p.m.

Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2063070 Primary Facility 04/26/2012 Drilling/Alteration No Violations Noted Falcon Rig 23. Drilling ahead on air. Walked site. Containment in good condition. Tear observed in front - noted this to Company Man. No violations observed at this time.
2078761 Primary Facility 06/21/2012 Follow-up Inspection Violation(s) Noted Spill of oil-base emulsifier. Spill caused from puncture of forklift. Operator reported to DEP in timely manner. Approx. 40 cuyds of soil removed from pad. Violations cited for drilling mud spilled on pad. Holes in secondary containment. Operator to properly removed residual drilling mud from site and report to DEP upon completion. A follow up inspection by DEP will be performed. See Inspection report for full details.
2143962 Primary Facility 02/27/2013 Routine/Complete Inspection No Violations Noted
2299620 Primary Facility 08/26/2014 Routine/Complete Inspection No Violations Noted Falcon Rig# 18 is the active drilling rig on the location. Protection, Pa. code 78.56.
2300070 Primary Facility 08/28/2014 Routine/Complete Inspection No Violations Noted
2307280 Primary Facility 09/20/2014 Routine/Complete Inspection No Violations Noted
2329100 Primary Facility 12/15/2014 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 12:00 in response to reported spills of (3-5) barrels of flowback water and (50) gallons of antifreeze.
2366968 Primary Facility 05/01/2015 Routine/Complete Inspection No Violations Noted
2452224 Primary Facility 2016-02-23 Routine/Complete Inspection No Violations Noted

2559658 Primary Facility 2017-02-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:10a.m., the cellar was full of gravel and there was no gas detected at the perimeter of the cellar.

2652097 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and no gas was detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
