2074460 2012-05-31 No Violations Noted THE SITE FOR THIS WELL IS UNDER CONSTRUCTION. E&S CONTROLS ARE IN PLACE.
2075945 2012-06-14 No Violations Noted John Ley, Water Quality Specialist, SWRO was on site to conduct an inspection. the site sign was in place and showed the proper permitting information. the E&S controls and BMP's were in place and constructed according to plan. The access road berms and soil stock pile had been stabilized. The pad was still under construction. The only issue was that the ICT drainage ditches at the bottom of the site slopes were filling up with soil boulders that had rolled down the slope as the pad was being constructed. These runoff ditches should be cleared.
2078633 2012-06-25 No Violations Noted John Ley, Water Quality Specialist, SWRO was on site to conduct a pre-operational inspection. The site sign was in place and showed the proper permitting information. The E&S controls and BMP's were in place and constructed according to plan. The access road berms had been stabilized. Additional work was taking place on the soil pile. The pad was still under construction.

The previous minor issue mentioned in the June 15th report dealing with the ICT drainage ditches at the bottom of the fill slopes was rectified. All the boulders have been removed and the ditches were dressed up. They will have rock added once the fill slope construction is completed.

2085898 2012-07-12 No Violations Noted TRIPPING IN HOLE TO RESUME DRILLING @ 1671'.
2085912 2012-07-19 No Violations Noted TOP HOLE COMPLETE AT 6793'.

2097427 2012-08-31 No Violations Noted LARGE ABOVE GROUND TANKS ARE BEING BUILT TO HOLD FRAC WATER. SITE IS IN GOOD CONDITION.
2097726 2012-09-05 No Violations Noted John Ley, WQS, SWRO was on site to conduct a routine inspection. At the time of the inspection, the well sign, E&S plan, and well permit were on site. BMP's installed were functioning according to the E&S plan. Hillsides were temporarily stabilized and had good growth on them. No accelerated erosion or sedimentation issues were observed on site or on the gated access road. Silt sock in the lower field will be removed to allow for mowing of the field.
The site had verticals drilled but drilling the wells was not complete. The site had temporary reservoirs constructed that were being filled with fresh water to be used to frac the Bakerstown Road site. When this operation has been completed, and the reservoirs removed, drilling will continue at this site.
