-site is still under construction. Rock contruction entrance is installed and entire pad surface had geotextile laid down with a layer of rock being laid on top of that. -road was dusty, but no mud remained. It appears to have been swept up and was probably a result of early stages of construction -Pit was being constructed. sides were sloped in and smoothed out, liner has not been installed yet -Site ID sign in place with all proper information filled in. -E&S plan on site. Controls in place and working adequately. -no issues on site at this time

1843859 2009-11-13 No Violations Noted -Site visited to attend a meeting between operator and concerned individuals working in or around the area. Concerned individuals seek cooperation from Chief to conduct in-stream monitoring of Bobs Cr.

-Site construction complete, all BMPs in place and working adequately -pit has been lined, NW corner of liner appears to be slipping down into pit. This was discussed with Mr. Mike Hritz of Cheif. It will be addressed before drilling starts. not currently an issue because there are no fluids in pit at this time -informed Mr. Hritz of complaint of mud on road. construction individuals should have been cleaning road -no issues on site at this time

1857105 2010-01-20 No Violations Noted Conducted Site Inspection with WQS Jessica Miller.

Met with Bill Swanson on site. Patterson Rig 255 set and cemented 2500' of 9 5/8" surface casing and was just finishing up with the BOP test. Rig is planning to drill into the Onondaga formation and then log and plug back to 7050' KOP. Bill accompanied Jessica and me on a site tour. Driller's log from Zigenfuss which drilled and set the 13 3/8" casing was not on location. All driller's logs need to be kept on site while drilling operations are still in progress. Site ID Sign and E/S plan are on site.

1857429 2010-01-20 No Violations Noted -Site inspected with OGI Travis Walker and accompanied by Company man, Bill Swanson.

-BOP test has just been completed and drilling is to commence shortly -E&S plan and Site ID site Remain Visible on site. E&S controls remain in place and working adequately. -Mr. Swanson indicated that the entire pad had been lined with felt and an impermeable liner. The pad is also angled towards the pit so all fluids should naturally flow into it. The pit has more than 2 ft of freebaord and liner remains intact with no signs of ptential tears or punctures. No tophole water was encountered to this date. -it was brought to Mr. Swanson's attention that due to the amount of snow on site and plowing mixing it with sediments, a large melting event could create a potential for sediments to leave the site and extra precautions should be taken then.

1860453 2010-01-28 Violation(s) Noted Met on site first with Tool Pusher Trey Villarreal.

Mr. Villarreal informed me of a spill of drilling cuttings that were leaking from the shakers approximatley 10' from the pit. I also noted a small spill from where the mud was transferred from a truck to the tanks. This is a small spill that just needs to be cleaned up. I later met with Don Henson, and Matt Barch, the Enviromental Compliance Specialists from Chief Oil and Gas. Inspected BOP Test from 1/20/10 Patterson Rig 255 is currently drilling an 8 3/4" hole through the curve at 6687' Rig will be moving to Somerset for the next well. Matt and I again went to inspect the spill at the shakers and discussed what steps would be taken to seal the area where the spill is coming from. A piece of rubber has been ordered to seal the leak to prevent future cuttings loss. Also, there water that is being sprayed onto the location due to the high wind in the area. The discharge line needs to be set closer to the pit liner so that the wind can not carry the water out of the pit.

The spill that was noted in inspection # 1860453, dated 1/28/10, has been cleared from the well pad and placed in the impoundment pit.

1865752 2010-02-23 No Violations Noted -Site inspected with OGI Travis Walker. Mr. Walker was conducting a follow-up to a previous NOV that he wrote

-The well is complete and all equipment has been moved from the site. A front end loader was on site and being used to move snow around on the western side of the pit. - The material that was noted previously by Mr. Walker had been scraped up and placed in the reserve pit. The was a small amount left around the cellar. Mr. Walker later brought this to the company's attention via phone call. -A lot of snow has fallen on-site. In a pile of plowed snow on the SE corner of the site there is a large sack of barite that appears to have been caught in the plowing process and mangled. The barite should be removed from the site prior to the snow melting. -There are currently no issues on site.

1869414 2010-03-12 Violation(s) Noted - Inspection conducted after a long period of warm weather and during a rain storm.

-BMPs remain working adequately. There are no signs of sediment laden runoff leaving the site. A field meter was used to measure TDS of some of the waters moving on site. One stream, just below the large snow pile on the SE corner of the pad carries a 800-900ppm TDS. waters running off of the main road and below the pad carry waters of 100-150 ppm. The elevated TDS does not constitute a violation at this time. - about 3/4 of the way across the fill side of the pad there is a pile of raw sewage about 2' in diameter (photos taken). -rathole has been filled in, pit remains within regulation.

1870561 2010-03-18 No Violations Noted - Inspection conducted as follow-up to 3-12-2010 inspection revealing violations of the Solid Waste Management Act.

-The site has been cleaned up since last inspection. All of the PVC pipe, and other debris have been removed. The large sack of barite that was stuck in the snow pile has also been removed from the site. There is still slight evidence of raw sewage. The majority of the solids have been cleaned up. -Seed and mulch remains in place. no sediments are leaving site -there are no other issues on site at this time.
