| 1378305284 | 2013-09-04 | I am on site today to review remediation measures for the spill which occurred 9/03/2013. I found that a vacuum truck has been vacuuming fluid since the spill occurred around noon on Tuesday. The fluid which is coming from under the liner and off the rock pad continues off the site 200 ft then down over the hill. The vacuum truck has been on site and has pulled up almost 100 bbls of fluid at this time. I contacted Deputy Chief Worstall to update on my findings. He said that I should request assistance from ODNR Engineers and have Kyle Baldwin collect samples for the lab. I collected quantab samples which show chlorides over 6000 PPM. Next I collected photos and will merge them with a spill report for Columbus. The Company Man on site when I arrived was Randal Stogner and was leaving for his days off at 7:00 am. There is no cement information on site and I requested a copy from the arriving Company Man Matt Knight. | ||||

| 473019532 | 2014-03-30 | On the day of my inspection I was notified by Kris Andersen of ALL Consulting that there was a spill of oil based mud while rigging down was taking place. One plug was inadvertently left off on the discharge line of two frac tanks on the evening of 3/29/30 with ice/solids in line from contents of tanks. Today at noon solids control hand Bob Baldwin was backing in a vacuum truck when the ice/solids plug broke free spilling approximately 12 total barrels of oil based mud. 10 barrels inside containment and due to a tear in the liner underneath the drill mats 2 barrels left containment. Upon my arrival I witnessed that drill mats were being lifted up and power washed off and cedar chips were being applied outside containment to soak any residual oil based mud. Owner/Operator shall pull up remainder drill mats isolate and vacuum inside containment and then roll back liner and clean up any pools of contamination. Once pools are cleaned Owner/Operator shall dig down with backhoe and remove contaminated stone and notify inspector so area can be re-inspected prior to adding clean fill. | ||||
| -687135898 | 2013-09-12 | Met Deputy Chief Bob Worstall on site with Ross Kirtley to review improvements made to the liner and the containment system in the cellar. The wells which are not being drilled are closed off and the well being drilled is contained from the Tin Horn to the containment liner. Bob and I also reviewed the spill which occurred 09/03/2013. The containment system has been improved to catch more of the chlorides. During our inspection we found that the fluid running over to be captured has almost stopped. The containment system to capture the chlorides is to remain in place until ODNR determines it is safe to remove it. Deputy Chief Worstall advised Ross Kirtley that he would pass along the findings of our inspection to Chief Simmers and he would make the determination if drilling could continue. | ||||
| -713885669 | 2013-09-04 | I am on site today to see what has been done to remediate the spill which occurred 09/03/2013 I found that no equipment has been on site. The only thing done has been to dig a small shovel wide ditch and use a vacuum truck to pull up the fluid it generates. Next I collected two low range chloride quantab samples. Both of the readings peaked the low test strips. Hal Miller tested the same fluid and found that the range exceeded the 6000 PPM range. I contacted Deputy Chief Worstall and advised of my findings. He advised me to call Kyle Baldwin and Rick Triple and see if they could lend some assistance. I remained on site to meeting with the above people and fill out spill reports and download photos for Columbus. After meeting with Blake, Rick, and Kyle we all agreed more information and results from the lab would be needed to decide on remediation. Blake and Rick Triple are to develop a short term plan and go over that with Gulfport and Nomac 310. | ||||
