| My visit to the well today was twofold. The city of North Royalton had requested that Bass Energy relocate the access road to this well due to construction on city property adjacent to the original access road. Bass Energy notified me that this was accomplished in late August of 2012. The access road was constructed to best management practices for site construction and meets requirements of 1509:09-9-9-05. I conducted a well inspection while on location. Well head location is clean and in good operating condition. There is 180 Pounds pressure at the well head to the feed line. The tank battery has two (2) 140 Barrel storage tanks and caps are secure and in good condition. There is one  (1) fifty barrel brine tank. There is a separator unit and a gas feed line unit. Fencing is chain link eight feet tall with three strands of barbed wire surrounding the top. Well is in compliance. No violations found.

| I inspected this producing well location as part of routine inspection activity. At the time of this inspection, I noted the following: a wellhead with pump-jack system and associated tank battery in separate chain-link enclosures located in the back portion of a residential parcel adjacent to the public library. The individual enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. The pump-jack supports were solid. The stuffing box fitting was secure with no leaking or problems. The bridle attachments were secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment was dry and free from any signs of spills, leaks, or other problems. The access road is comprised of a gravel-paved, dedicated road that enters from Wallings Road, and travels 900 feet to the well and battery.  I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity.

The load-lines were secure, plugged and locked. The secondary containment was intact and free from signs of leakage. The enclosure gates were locked and secure. The gravel-paved access road was level and dry with no signs of wash-outs or problems. I walked and inspected the areas surrounding the well and battery and found no overt or observable signs of environmental impact. Thre production reports were current. The owner was current with bonding and insurance.

Process safety equipment is present. Fittings and connections are secure and free from signs of leakage or problems. Tank hatches are closed and secure. The access road is dry, level, and well drained with no signs of erosional deficit. At the time of this inspection, I observed the following deficiencies: The electrical box is not lock protected, ID signage is lacking required information. There is no evergreen screening which is required as part of site restoration, unless waived by the landowner. Warning signage is incomplete.  I have notified Bass Energy that these deficiencies must be corrected as soon as practicable or by 06/29/2015.  Please contact Inspector Stephen Tompkins at 330-907-4647 for questions regarding this inspection.

| My visit to the well today was twofold. The city of North Royalton had requested that Bass Energy relocate the access road to this well due to construction on city property adjacent to the original access road. Bass Energy notified me that this was accomplished in late August of 2012. The access road was constructed to best management practices for site construction and meets requirements of 1509:09-9-9-05. I conducted a well inspection while on location. Well head location is clean and in good operating condition. There is 180 Pounds pressure at the well head to the feed line. The tank battery has two (2) 140 Barrel storage tanks and caps are secure and in good condition. There is one  (1) fifty barrel brine tank. There is a separator unit and a gas feed line unit. Fencing is chain link eight feet tall with three strands of barbed wire surrounding the top. Well is in compliance. No violations found.

At the time of this inspection, I noted the following: a wellhead with pump-jack system and associated tank battery in separate chain-link enclosures located in the back portion of a residential parcel adjacent to the public library. The individual enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. The pump-jack supports were solid. The stuffing box fitting was secure with no leaking or problems. The bridle attachments were secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment was dry and free from any signs of spills, leaks, or other problems. The access road is comprised of a gravel-paved, dedicated road that enters from Wallings Road, and travels 900 feet to the well and battery. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity.
