| I was on location to check the status of this recently completed well.  The well was hydraulically fractured on 12/12/12.  Hagen Well Services and Wade Pol Contracting were on location with Don Toth (Equity) preparing to run tubing into the hole.  The wellhead was still hooked up to a 300 barrel flowback tank and 2.375 tubing was being unloaded and measured.  There were six 300 barrel frac tanks on location.  The pits were still open, in good condition, and had plenty of freeboard.  W. Pol cotracting told me they would solidify and close the pits on 12/20/12.  No violations.

| I was on location with John Murphy (Equity Oil & Gas) to witness the cementing of the 4.5 production casing.  I visually inspected the used pipe and it was in good condition.  John Murphy gave me a copy of the invoice showing that the pipe had been drifted and pressure tested to 4000psi.  Poulson drilling ran 122 joints of 4.5 procuction casing with a 10' pup and shoe on the bottom and set at 4395' (KB) with a 9' landing joint on top.  Petroset rigged up, broke circulation, mixed and pumped 200 sacks of Class A cement, dropped the latch-down plug, and displaced the plug with 70 barrels of freshwater.  The plug landed at 1500psi and held.  See casing report for more details on this job.  No violations.

| I was on location with Kevin Perry to inspect the status of this recently completed well.  The well was equipped with rods and tubing, but the pumping unit had not been installed.  The pits had been closed and the area had been graded.  The topsoil was stockpiled on the west side of the location and will need to be spread, seeded, and mulched in the spring.  The lease road was in good condition.  It appears that the flow lines are being installed and run towards a common tank battery for the Webb Unit #s 1-3.  No violations.

0850740729 2012-12-13 Completion Testing No Violations I was on location to check the status of this recently completed well. The well was hydraulically fractured on 12/12/12. Hagen Well Services and Wade Pol Contracting were on location with Don Toth (Equity) preparing to run tubing into the hole. The wellhead was still hooked up to a 300 barrel flowback tank and 2.375 tubing was being unloaded and measured. There were six 300 barrel frac tanks on location. The pits were still open, in good condition, and had plenty of freeboard. W. Pol cotracting told me they would solidify and close the pits on 12/20/12. No violations.
1095924344 2012-11-27 Drill / Deepen / Reopen No Violations I was on location to witness construction of the drilling pits. W. Pol contracting was digging the pits. The soil was clay rich and bedrock was not encountered. Poulson Drilling was rigging up and moving equipment onto location. The pits were properly lined and a plastic culvert was placed in the cellar to contain fluids discharging to the pit. Poulson Drilling is planning to spud the well tomorrow morning. No violations.
1145844655 2012-11-29 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the surface casing. The drilling pits were in good condition and had plenty of freeboard. There were no reported circulation problems while drilling the surface hole. Poulson Drilling ran 10 joints of new, 23lb/ft, 8.625 surface casing plus a 10 pup joint to a depth of 410 at the kelly bushing (KB) with 2 centralizers. Petroset rigged up, broke circulation, mixed 190 sacks of cement, and pumped the slurry with 50lbs of cottonseed hulls ahead. Good returns were achieved at the surface and held for 30 minutes. 1354288422 2012-11-28 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the conductor casing. Poulson Drilling ran 3 joints of new, 32lb/ft, 11.75 conductor casing to a depth of 116 at the kelly bushing (KB). Petroset rigged up, broke circulation, mixed 200 sacks of cement, and pumped the slurry with 50lbs of cottonseed hulls ahead. Good returns were achieved at the surface and held for 30 minutes. 1354803634 2012-12-03 Drill / Deepen / Reopen No Violations I was on location with John Murphy (Equity Oil & Gas) to witness the cementing of the 4.5 production casing. I visually inspected the used pipe and it was in good condition. John Murphy gave me a copy of the invoice showing that the pipe had been drifted and pressure tested to 4000psi. Poulson drilling ran 122 joints of 4.5 procuction casing with a 10 pup and shoe on the bottom and set at 4395 (KB) with a 9 landing joint on top. Petroset rigged up, broke circulation, mixed and pumped 200 sacks of Class A cement, dropped the latch-down plug, and displaced the plug with 70 barrels of freshwater. 1361907755 2013-01-24 Production Wells No Violations I was on location with Kevin Perry to inspect the status of this recently completed well. The well was equipped with rods and tubing, but the pumping unit had not been installed. The pits had been closed and the area had been graded. The topsoil was stockpiled on the west side of the location and will need to be spread, seeded, and mulched in the spring. The lease road was in good condition. It appears that the flow lines are being installed and run towards a common tank battery for the Webb Unit #s 1-3. No violations.
1371826832 2013-05-16 Preliminary Restoration No Violations I was on location to check the preliminary restoration status of this well. There is no drilling equipment on location. The pits have been filled and the site has bee graded and seeded around the pumping unit and drilling location. Preliminary restoration is approved. No violations.
