| Drill / Deepen Reopen
| I inspected this producing unit as part of regular site check and inspection activity. The well was completed on 2/21/2008. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located west and adjacent to the wellhead. The battery is comprised of 2 steel 100-barrel production tanks, and a horizontal separator. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area of the well is generally level with a slight topographic drop to the west. Surface draining appears to be controlled by local infiltration.  I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a gravel-paved access road which is partially shared with the house at 29750 Havard Rd. The short access road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are heavily wooded with one single-family residential home bordering to the south, and no other residential strucutres within eyesight.

| I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead and tank battery enclosures were locked and secure. Tank lids were closed and sealed. Load lines were closed, plugged, and secured with locks. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery which are comprised of meadow grasses and lightly wooded areas, and found no evidence of problems or impact to the surrounding greenscape attributable to the on-going production activity.

| I inspected this producing well as part of routine inspection activity.  The well and associated tank battery are located in the back of a single-family residence.  At the time of this inspection, the enclosure gates at the wellhead and battery were locked and secure. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. Areas around the wellhead and battery were comprised of grassy vegetation and wooded areas. I inspected the surrounding areas and did not observe any evidence of problems or impact. I observed the following maintenance deficiency: A portion of the battery enclosure fence is broken down and requires repair/replacement. I also observed the following safety deficiency: The production tank lids were old style hatch lids which do not seal and are not compliant with OAC 1501:9-9-05(E)(4)(d).  Hatch lids must be replaced with lids that have functioning seals and must be closed and secure . These safety deficiencies must be addressed by 01/16/2016. Please contact Inspector Stephen Tompkins at 330-907-4647 regarding this inspection.

| I inspected this well location as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted that the tank lids were not capable of sealing as required by OAC 1501:9-9-05(E)(4)(d). At the time of this inspection, the lids had been replaced and were in compliance. I inspected the rest of the production elements and observed the following: fittings and connections at the wellhead and battery were secure and free from leakage or problems. The secondary containment was intact and the interior was dry. The enclosure gates were locked and secure. The shared access road was in good condition.

0594593159 2017-10-04 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity.

1403103293 2014-06-18 UrbanProduction Wells No Violations I inspected this producing unit as part of regular site check and inspection activity.

1846604316 2015-12-11 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity.
1923980459 2009-11-12 Preliminary Restoration No Violations no violation.
