| Inspection this day found well being produced by landowner, an unregistered operator, well has jack and is pumped occasionally, supplying gas to landowner.  Tank is 100 bbl, overgrown, needs IDs, poor dike.  Landowner took lease away from all producers in 1993 - he thought he also got wells.  Would be idle well if landowner stopped using gas.  I turned in inspection on 4/6/04 yet not showing up on 3-D topo as a well inspected.  This location a mess - landowner dumping saltwater/occasional oil in creek.  Someone needs to come and take responsibility.

| This is a Joe Flarerty deal - well is being produced illegally by landowner who is draining saltwater and oil, no ID's, well has jack, JOE - update lat/long - landowner has actually sold oil off lease but couldn't collect his money - is using well for house gas - also using 7862 - I advised him to see if he could buy wells from Joe and gave him papers to get well in his own name.  I am going back in week or so to get everything straightened out.  Well 7862 says it is plugged, but it's not..  Tank says Pioneer Western at top.

| Inspection this day found well being produced by landowner, an unregistered operator, well has jack and is pumped occasionally, supplying gas to landowner.  Tank is 100 bbl, overgrown, needs ID's, poor dike.  Landowner took lease away from all producers in 1993 - he thought he also got wells.  Would be idle well if landowner stopped using gas.  I turned in inspection on 4/6/04 yet not showing up on 3-D topo as a well inspected.  This location a mess - landowner dumping saltwater/occasional oil in creek.  Someone needs to come and take responsibility.

In Beverly) says they don't want it - somebody can have it to produce if I find someone.  This NOV resolved in 8/06 due to Epler using gas from it - can't give violation.  Joe - noticed this in my box at office requesting another look-at.

1113814598 2004-04-06 Production Wells No Violations This is a Joe Flarerty deal - well is being produced illegally by landowner who is draining saltwater and oil, no IDs, well has jack, JOE - update lat/long - landowner has actually sold oil off lease but couldnt collect his money - is using well for house gas - also using 7862 - I advised him to see if he could buy wells from Joe and gave him papers to get well in his own name. I am going back in week or so to get everything straightened out. Well 7862 says it is plugged, but its not.. Tank says Pioneer Western at top.
1395912859 2006-08-22 Production Wells No Violations Inspection this day found valves open, well has jack, motor missing. Land now owned by Rick Walters, et al - talked with him at length and he could like to adopt well as domestic exempt or as a producer. Will decide when he sees what John Epler, owner of part of the parcel of land and owner of 7874 is going to do. They have a hunting trailer they will use the gas in and they may adopt other 2 wells nearby (#5154 and #5060) and try and produce all with a bond. Gave him paperwork and explained what he needed to do. Was going to discuss with his partners in ownership but should follow thru. Will resolve NOV as landowner now using.
1490113353 2017-03-21 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter wellhead, and 1 1/2 inch diameter production tubing. The valves on the wellhead are open at the time of inspection. The tank battery for this well is equipped with a 100 barrel steel tank and a vertical separator. There are no issues to report at this time and the well is in production at the time of inspection.
