During my inspection I witnessed Formation Cementing nipple up to the tubing and pressured up on the hydraulic setting tool to fully set the plug and release the hydraulic setting tool so the tubing could be tripped back out of the hole. Bishop well service tripped the 2.875 inch diameter tubing out of hole. The setting tool came to surface with the mandrel of the plug still attached indicating that there is a hole in the middle of the plug that was set at 7578 feet within the 5.5 inch diameter production casing.  Bishop well service will trip back in to clean the hole and repeat the process.

Peak Completions assembled the hydraulic setting tool and the bridge plug on to the 2.875 inch diameter tubing and the tubing was tripped back into the 5.5 inch diameter production casing. The plug set prematurely at 7578 feet. Bishop Well Service made an exhausted attempt to push the plug further down hole. The plug would not move. Operations will continue tomorrow 4/4/17.

Cactus Wellhead nippled down the wellhead and nippled up their blow out preventer dressed for 2.875 inch diameter tubing.  The well consisted of 2.875 inch diameter tubing set on a packer at 6519 feet.

| The purpose of my inspection was to check in on the process of setting a bridge plug to satisfy unit boundary requirements. Wise Coil Tubing is on location along with Artex. Wise attempted to trip in hole to tag total depth and then set a bridge plug. They were unsuccessful in doing so. Prior to my arrival the coil tubing made it to 11,100 feet in the lateral and got hung up and could not go any further. During my inspection Wise Coil Tubing was rigging down to move off of location. Artex plans to bring a service rig on location to clean out the well to make it possible to get out in the lateral far enough to set a bridge plug at 11,206 feet.

During my inspection I witnessed bishop well service tripping 2.875 inch diameter tubing in hole. The tubing was assembled with a concave bottom junk mill bit that will assist in pushing the failed bridge plug remains from 4/3/17 and any other debris within the 5.5 inch diameter production casing out passed 9000 feet so a new bridge plug can be set at the desired depth.

1490108445 2017-03-21 Work Over Reconditioning No Violations On location to witness Bishop Well Service along with Peak Completions release the packer on the 2.875 inch diameter tubing and remove the tubing from hole. Bishop attempted to release the packer and began to remove the tubing from hole. The packer was still hanging up on the first 12 joints. The packer became free and the tubing was free to be removed from hole. I witnessed the first 31 joints being removed.

1491318024 2017-04-03 Work Over Reconditioning No Violations Peak Completions, Bishop Well Service, Kilbarger, Formation Cementing, and Artex are all on location to attempt to set a cast iron bridge plug at a measured depth of 8850 feet in depth within the 5.5 inch diameter production casing. During my inspection I witnessed Bishop Well Service pulling the 2.875 inch diameter tubing from hole. Peak Completions assembled the hydraulic setting tool and the bridge plug on to the 2.875 inch diameter tubing and the tubing was tripped back into the 5.5 inch diameter production casing. The plug set prematurely at 7578 feet. Bishop Well Service made an exhausted attempt to push the plug further down hole. The plug would not move. Operations will continue tomorrow 4/4/17.
1563375881 2017-03-20 Work Over Reconditioning No Violations On location today to meet with Artex Oil Company to discuss setting a bridge plug for purposes of eliminating a unit boundary overlap on an additional horizontal well that Artex plans to drill. During my inspection Bishop Well Service rig was rigging up. Cactus Wellhead nippled down the wellhead and nippled up their blow out preventer dressed for 2.875 inch diameter tubing. The well consisted of 2.875 inch diameter tubing set on a packer at 6519 feet.

1917367438 2017-03-23 Work Over Reconditioning No Violations The purpose of my inspection was to check in on the process of setting a bridge plug to satisfy unit boundary requirements. Wise Coil Tubing is on location along with Artex. Wise attempted to trip in hole to tag total depth and then set a bridge plug. They were unsuccessful in doing so. Prior to my arrival the coil tubing made it to 11,100 feet in the lateral and got hung up and could not go any further. During my inspection Wise Coil Tubing was rigging down to move off of location. Artex plans to bring a service rig on location to clean out the well to make it possible to get out in the lateral far enough to set a bridge plug at 11,206 feet.
