| I arrived to this well and found River Valley Energy had removed the horse head to repair it. The well was cited for  1501:9-1-07 and 1509.22(A), well causing pollution and contamination. The oil that was spilled was absorbed with peat sorb. The peat sorb has been removed. This satisfies the compliance notice written on 7.17.18.

| I received a call from well tender Ron Ullman who works for well owner Geopetro. A stuffing box leak caused approximately 1/2 barrel of oil to escape. When I arrived, Ron had Peat Sorb spread on the oil. The oil spill was approximately 12 feet by 30 feet and did not enter into any water course. I asked Ron to remove the Peat Sorb tomorrow and add new Peat Sorb where needed. Pictures were taken during my inspection.

| Follow-up revealed all oil on the ground surface has been removed. Oil in the pond has been contained with oil booms and absorbent pads. I called the pumper and he advised me that with the flooding no work was performed yesterday but a crew will be working on it to day and depending on the weather the work should be completed the middle of next week.

| At 9:35 AM I called well tender Mr. Ron Fisher and attempted to leave a message. The phone number I dialed would not allow me to leave a message. The peat sorb that had been used to absorb the oil remains on the ground. The peat sorb will need to be removed before the compliance notice can be written off.

January 29, 2018 report claims there are three 100 barrel tanks at the tank battery there was iegible identification at the well head. There are three 210 barrel tanks and one plastic tank at the tank battery and the identification is not complete at the tank battery, ID is missing Owner name and ER# is wrong.
