| I inspected this well as a follow-up to Chiefs Order  #2014-202 for failure to maintain liability insurance. At the time of this inspection, but the well was not being produced commercially based on the closed flow-line connections. The was a concoil unit and associated tank battery (comprised of one 210-barrel steel tank, one 100-barrel steel production tank, with mechanical separator). The well and battery are located in a wooded portion of a larger, rural parcel.  Elements of the system appeared to be in disrepair and the flow-line connections were closed.  There was no identifying signage. There remained no active liability insurance on file for Plateau Energy Corp.

| My inspection found the 210 barrel that is stored next to the tank pad area is empty. The 100 barrel steel tank in use is full of fluid to within one foot of the top of the tank. The company has not yet obtained liability insurance at this time. I spoke with Charles Mosio and told him the 100 barrel steel needs to be emptied. I also told Charles this has been ordered to be plugged due to a forfeiture of lease on the property the well is located.

| Complaint# 4964, Mark Elliot. The tank battery was equipped with one 100 bbl steel tank. There was a 210 bbl steel tank on south side of the earthen dike and was not connected to anything. There was no identification posted at the tank battery area. Inspection of the well found the well is equipped with a concoil unit. There is a separator located 100 feet north of the well. The well is not in production at this time. The last reported production for this well was in 2011 which was 774 mcf of gas, 75 bbl of brine and no oil.  There is a unresolved Chief's order issued on 7-14-2014 for failure to maintain liability insurance. REMEDIAL WORK REQUIRED TO RESOLVE THE COMPLIANCE ISSUES: OBTAIN LIABILITY INSURANCE, AFTER LIABILITY INSUANCE IS OBTAINED PUT WELL BACK INTO PRODUCTION, ESTABLISH IDENTIFICATION AT THE TANK BATTERY AREA, SUBMIT PRODUCTION REPORTS FOR 2012 TO PRESENT. I spoke with Charles Moisio, he claims he acquired all the wells from Plateau Energy from his family after Bob Anderson passed away. He also said he is operating the wells under the bond for Plateau Energy. I told him to update the records [ address and phone number ] with the Division of Oil & Gas. I also told him of the violations listed above and the remedial work required to comply with 1509 of the Ohio Revised Code.

The was a concoil unit and associated tank battery (comprised of one 210-barrel steel tank, one 100-barrel steel production tank, with mechanical separator). The well and battery are located in a wooded portion of a larger, rural parcel. Elements of the system appeared to be in disrepair and the flow-line connections were closed. There was no identifying signage. There remained no active liability insurance on file for Plateau Energy Corp.

I spoke with Charles Mosio and told him the 100 barrel steel needs to be emptied. I also told Charles this has been ordered to be plugged due to a forfeiture of lease on the property the well is located.
1430742800 2015-03-18 Production Wells Violations Noted Complaint# 4964, Mark Elliot. The tank battery was equipped with one 100 bbl steel tank. There was a 210 bbl steel tank on south side of the earthen dike and was not connected to anything. There was no identification posted at the tank battery area. Inspection of the well found the well is equipped with a concoil unit. There is a separator located 100 feet north of the well. The well is not in production at this time. The last reported production for this well was in 2011 which was 774 mcf of gas, 75 bbl of brine and no oil. There is a unresolved Chiefs order issued on 7-14-2014 for failure to maintain liability insurance. REMEDIAL WORK REQUIRED TO RESOLVE THE COMPLIANCE ISSUES: OBTAIN LIABILITY INSURANCE, AFTER LIABILITY INSUANCE IS OBTAINED PUT WELL BACK INTO PRODUCTION, ESTABLISH IDENTIFICATION AT THE TANK BATTERY AREA, SUBMIT PRODUCTION REPORTS FOR 2012 TO PRESENT. I spoke with Charles Moisio, he claims he acquired all the wells from Plateau Energy from his family after Bob Anderson passed away. He also said he is operating the wells under the bond for Plateau Energy. I told him to update the records [ address and phone number ] with the Division of Oil & Gas. I also told him of the violations listed above and the remedial work required to comply with 1509 of the Ohio Revised Code.

The tank battery location still had the 210 BBL production tank onsite, that is being stored next to the tank battery and was empty. I spoke with Charles Mosio of Plateau Energy with regards to emptying the 100 BBL production tank and removing the 210 BBL production tank.

There is a 210 BBL production tank that has been disconnected from the flow line and is outside of the diked battery, that still needs removed from the site.
