| I received a letter from an attorney representing Rural Land Holdings LLC in regard to having this well plugged immediately. I visited the site to get an updated inspection in response to the letter. I found that there had been no work at the well site, and the well was still shut in and disconnected from the flow lines. There was a pool of water around the 4.5 inch diameter casing. The surface casing was not visible. I found gas bubbling up through the water. I tested the water for chlorides and it came back negative. I called Dale Smith to report the leak and he said that he would send a crew out to dig up the surface casing and attempt to stop the gas. I also sent an email to Tom Vosick with M&R Investments to report the issue and ask that they get this well plugged as soon as possible. There was a compliance notice issued on 8/24/17 to plug the well. No permit has been issued at this time.

| On the day of my inspection I found that the cellar had been dug out enough to expose the surface casing nipples. The cellar was still full of water and there was still gas bubbling from the surface casing nipple and from the production casing wellhead around the hammer union. The path back to the well had been recently mowed, and now that the grass was short I found a partial string of tubing laying on the ground near the well. I called Dale Smith to ask what their plan was. He said that there is annular pressure on the surface casing and they cant open anything up to work on the well until they get the flow line reconnected so they could sell off the annular gas. He also said that they compromised the integrity of the threads on the hammer union when they were trying to tighten it up on their last visit. Once they get the gas sold down they are going to replace the rubbers in the wellhead. Dale gave me a time frame of two weeks because his roustabout is on vacation until after Memorial Day.

The valves at the wellhead were closed and there were no signs of any vehicle traffic in an attempt to service the well for production. To this date no plugging permit has been issued for this well. I called Tom Vosick with Diversified to ask what the plans were for this well. He did not have any information as far as their plans to either plug or produce the well. Tom said that he would look into the permit numbers that I gave him and get back to me. He said that they sounded like wells that should be placed into their plugging program.

| On the day of my inspection I found that the well was equipped with a plunger lift system. The tubing and casing flow lines were both disconnected, and all of the valves were closed. I did not find any leaks at the wellhead, and I didnt find any signs of contamination in the surrounding soil. There was no apparent access route to the wellhead. The well was located in an open area surrounded by forest and agricultural fields. I found no vehicle, atv or foot path to access the well. The production equipment was located along the gravel drive used to access the property. It consisted of one 210 barrel steel tank, one 70 barrel steel tank, one horizontal separator and a drip system. All of the equipment was rusted and in a general state of neglect. It was difficult to get into the dike, which was heavily overgrown with vegetation. I did not notice any leaks at the tanks, but the vegetation was too thick to make a thorough assessment. There was no identification posted at the production equipment or the wellhead. There has been (0) zero barrels of oil and (0) zero cubic feet of gas reported for this well since 2012. The well is not making domestic gas, as the flow lines are disconnected and the structures on the property are abandoned. This well is idle and incapable of production in its current state. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

I reported this issue to M&R Investments on 3/28/19 by phone, and I was told that they would have a crew out the next day to resolve the gas leak. It has been 14 days since my initial contact with M&R regarding the gas leak, and nothing has been done yet. No soil has even been dug out from around the wellhead to expose the source of the leak. The gas leak must be corrected within 5 days of the receipt of this compliance notice.

He did not answer so I left a voice message.
