•All required paperwork present on site; site plans were in poor condition.
 +
•Vegetation along the uphill side of the access road was sparse and patchy in many areas.  No violation, remediation recommended.
 +
•ATV tracks, an empty case of beer, and other evidence were found suggesting that the site is being used for recreational activities.  No violation, increased site security recommended.
 +
•Twelve wells were observed on site; two additional wells are recorded in eFACTS under the site number and name but are geographically closer to the Oliver East pad and are reported as never drilled in GIS.

2515578 Primary Facility 2016-06-02 Routine/Complete Inspection No Violations Noted On 6/2/16, I performed routine complete inspection for the Oliver West site and made the following observations:

•All required paperwork present on site; site plans were in poor condition. •Vegetation along the uphill side of the access road was sparse and patchy in many areas. No violation, remediation recommended. •ATV tracks, an empty case of beer, and other evidence were found suggesting that the site is being used for recreational activities. No violation, increased site security recommended. •Twelve wells were observed on site; two additional wells are recorded in eFACTS under the site number and name but are geographically closer to the Oliver East pad and are reported as never drilled in GIS.
