| On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place for the 9.625 inch diameter casing gauge read 280 pounds per square inch, the 13.375 inch diameter casing gauge read 0 pounds per square inch.  Production equipment included a separator, Gas Processing unit, line heater, and 4- 400 barrel purge tanks shared with 3401320690, 3401320691, 3401320697. Identification tags were present and production was up to date.

| The well is currently equipped properly to be considered capable of production. The well appears to be free flowing currently with no artificial lift technology in place. The well is piped over to a gas processing unit and then routed to sale and to the tank battery. The tank battery contains six 400 barrel production fluid storage tanks. Beside the diked area is two vertical separators and a contact tower. The production records in RBDMS are current to 2017.

Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0112127320 2012-11-21 Production Wells No Violations On the day of my inspection the well was being flared. Company man John Shepherd present. No violations were noted.
0276793991 2012-06-28 Drill / Deepen / Reopen No Violations Had Drilling on site to spud conductor hole.
0526387641 2012-08-15 Drill / Deepen / Reopen No Violations Total depth has been reached@16,490 should be ready to run and cement 5.5 in place later this week.
0855988309 2013-01-24 Production Wells No Violations On the day of my inspection there some excavating work going on. No violations noted.
1341428399 2012-07-02 Drill / Deepen / Reopen No Violations Stopped by drill site for update they were setting conductor pipe there was 300 of 20 casing set by HAD Drilling and mouse hole was also set and backhoe was on site to dig cellar and set and then we cemented the bottom of tin horn to ensure of no ground water contamination.
1354565587 2012-11-21 Completion Testing No Violations Well is being flowed back at current time. Fesco is flowback group. Company man John Sheppard is at site and there is currently two flow back stacks. There is two flares about 75-100 tall on each stack. Well is currently at the 30 Million Cubic feet per day rate. There is 5400psi at well head. There were no violations at site.
1456149972 2018-05-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position.
1587163242 2017-07-19 Production Wells No Violations The well is currently equipped properly to be considered capable of production.
1635880301 2018-01-16 Production Wells No Violations
1679828096 2013-01-02 Production Wells No Violations On the day of my inspection there was no activity on the site. Well is capable of production. No violations noted.
1717885003 2012-09-25 Completion Testing No Violations Inspection revealed that crew was rigging up flow back iron from wellhead to tanks they plan on flowing well back in the next couple of weeks.
1810047827 2014-06-19 Production Wells No Violations On the day of my inspection the well had all necessary equipment to be capable of production. No violations noted.
1825241374 2014-10-23 Preliminary Restoration No Violations This site has proper erosion and sediment controls preliminary restoration on this site is complete.
1976008929 2013-12-16 Production Wells No Violations On the day of my inspection this well was capable of production. No violations noted.
2110647618 2012-08-16 Drill / Deepen / Reopen No Violations On site to witness running and cementing of production casing.
