I informed Chief HSE representative Joshua Gresch of my findings. He immediately had a suction truck begin vacuuming the puddles on the pad surface on which sheen was observed. The flat-bed truck with sheen on the bed was being cleaned with adsorbent materials. The barrels stored on the truck may be empty, but if not will be moved to an appropriate storage location within containment. No violations were cited during this inspection.

| Arrived onsite for a routine inspection at 12:04; the weather was ~30°F and breezy. The gate was open when I arrived.

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with gravel. The Baumunk 1H annulus was plumbed to an ~350 gallon poly tank and the Czop 1H and 2H were plumbed to a 550 gallon poly tank. The tanks were not on additional secondary containment. There were 3 GPUs, 3 produced water tanks, and 1 fresh water injection tank onsite and in secondary containment. It did not appear that the fresh water tank was labeled.

Two detention basins were recently installed/modified. These areas were not yet vegetated but they were temporarily stabilized with straw and erosion control matting. The matting was rolled in several areas and may need to be repositioned (Photos #3-6). These areas also had CFS and straw bales as E&S  controls. Other areas of the pad were well vegetated and no E&S  issues were noted.

No detectible gas at polytank. Left location 9:50. Gates were open when I arrived and two empty trucks were on location.

At the time of my inspection, Czop 1H was going on line. A sand separator and heating unit were staged near the well head so that high initial pressure (4000 psi) could be dropped back and sent to production units on site. Left location @ 14:10

2515805 Primary Facility 2016-09-01 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions and conditions on the well pad after installation of the conductor casings. No Chief representatives were present during the inspection.

Temporary erosion and sediment control BMPs are in place and in good repair. Site vegetation is <70% in many areas. No evidence of accellerated erosion or sedimentation was observed.

Two conductor casings have been installed. No evidence of spills such as staining, odor, or petroleum sheen was observed.

No violations were cited during this inspection.

2529595 Primary Facility 2016-10-20 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. A drill rig is on site. All chemicals, oils, and other fluids were observed to be stored within containment. No evidence of spills such as sheens or staining of the containment or pad surface was observed. Electrical conductivity of puddles on the well pad was screened and ranged from 250 - 900 uS/cm.

No violations were cited during this inspection.

2538723 Primary Facility 2016-11-22 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. A drill rig is on site. All chemicals, oils, and other fluids were observed to be stored within containment. No evidence of spills such as sheens or staining of the containment or pad surface was observed.

A tear was observed in the liner of containment being used for the storage of a number of barrels of fluids and pallets of dry material. The liner should be patched to ensure the proper functioning of the containment in the event a spill or release of the stored materials occurs. The Department requests that Chief provide photographic docuemtnation that this tear has been repaired.

No violations were noted during this inspection.

2568650 Primary Facility 2017-03-07 Routine/Complete Inspection No Violations Noted Routine inspection conducted during well stimulation activities. During the inspection, several housekeeping items were identified, as described in this report. I informed Chief HSE representative Joshua Gresch of my findings. He immediately had a suction truck begin vacuuming the puddles on the pad surface on which sheen was observed. The flat-bed truck with sheen on the bed was being cleaned with adsorbent materials. The barrels stored on the truck may be empty, but if not will be moved to an appropriate storage location within containment. No violations were cited during this inspection.
2572066 Primary Facility 2017-03-16 Routine/Partial Inspection No Violations Noted Inspection conducted to assess site conditions during well stimulation activities. Site covered in snow, which is deep in between equipment and in containment areas. No evidence of spills such as odors, staining, or sheen was observed. Stimulation activities were just starting after having been halted for the previous two days due to a winter storm. Chief representative Chris Bass was present during the inspection. No violations were cited.
2574872 Primary Facility 2017-03-24 Routine/Partial Inspection Pending Inspection conducted to assess conditions during well completion activities. Chris Bass of Chief was present.

Several areas with elevated conductivity were noted. Trash along the edge of the site was also observed. Mr. Bass indicated the puddles and trash would be removed.

Since the source of the elevated conductivity is unknown until lab results of a collected sample are received, violations are pending.

2719588 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted Three wells staged for production phase. Three each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. Methanol tank located near well heads. The lower Infiltration basin may require further engineering for proper drainage. No violations noted.
2827040 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:04; the weather was ~30°F and breezy. The gate was open when I arrived.

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with gravel. The Baumunk 1H annulus was plumbed to an ~350 gallon poly tank and the Czop 1H and 2H were plumbed to a 550 gallon poly tank. The tanks were not on additional secondary containment. There were 3 GPUs, 3 produced water tanks, and 1 fresh water injection tank onsite and in secondary containment. It did not appear that the fresh water tank was labeled.

Two detention basins were recently installed/modified. These areas were not yet vegetated but they were temporarily stabilized with straw and erosion control matting. The matting was rolled in several areas and may need to be repositioned (Photos #3-6). These areas also had CFS and straw bales as E&S controls. Other areas of the pad were well vegetated and no E&S issues were noted.

The Department recommends repositioning and securing
