| A follow-up inspection was conducted o 8/31/11to determine if the violations noted during a 6/21/10 (1906058) inspection had been resolved.  The pit was still located at the site and the liner had multiple holes in it.  Also, the orange safety fence had fallen down in one area.  The violations cited in June 2010 for 25 PA Code §78.65(2) - Failure to restore site within 30 days of permit expiration when well not drilled that has not yet been resolved one additional violation has been cited during this inspection for 25 PA Code §78.56(a)(3) ¿ Pits and tanks shall be designed, constructed and maintained to be structurally sound and reasonably protected from unauthorized acts of third parties are still outstanding.  You had indicated in your response to the Notice of Violation 7009 1680 0000 0578 4803 that this pit had been removed.  This is obviously not the case.

| The Department conducted a follow up inspection on 8/5/14 @ 1220 hours in Hector Twp., Potter Co.  The weather is cloudy, 75 degrees with thunderstorms in the immediate area. The onsite contractor, Dyne Construction, is using an excavator to backfill and regrade the pit area. The liner has been removed, and the contractor stated “the cuttings were spread on a field past WCP6”. A bale of mulch material is visible onsite. The contractor stated “Steve [Dyne] has the seed mix and is not onsite today”.  Violations cited on 6/21/10 and 3/7/12, Inspection Records #1906058 and #2048796, remain outstanding. No new violations issued during this inspection.

| A routine, post-plugging, inspection of this uncompleted conventional well was conducted December 6, 2018. At the time of inspection, the well has a 7" x 2" candy cane vent pipe 4' high with a marker plated erected atop 7" surface casing as a plugged well monument. There was no visible or audible venting at the vent pipe. No gas was detected at the mouth of the vent with an Altair 5x gas meter. No discharge, venting, or bubbling was observed outside of the 7" casing. No obvious issues were observed during this inspection.

| A compliance evaluation inspection was conducted on May 8, 2012 to determine if the violations noted during previous inspections have been addressed.  A June 21, 2010 inspection (1906058) resulted in a violation of 25 PA Code ᄃ78.65(2) - Failure to restore the site within 30 days of permit expiration when it was determined the well was not drilled with due diligence and a violation of 25 PA Code ᄃ78.56(a)(3) ï¾– Pits and tanks shall be designed, constructed and maintained to be structurally sound and reasonably protected from unauthorized acts of third parties.During the March 7, 2012 (2048882) inspection no site identification sign, no drilling permit and no well tag were observed at the site.    The Erosion Control Plan at the site is incomplete.  The plan is illegible and there were no records on site documenting the maintenance program of BMPs and the inspection of BMPs on a weekly basis and after each storm event. Today's inspection showed nothing has been done so far to correct these violations.  No new violations were cited during this inpsect

The weather is cloudy, 75 degrees with thunderstorms in the immediate area. The onsite contractor, Dyne Construction, is using an excavator to backfill and regrade the pit area. The liner has been removed, and the contractor stated “the cuttings were spread on a field past WCP6”. A bale of mulch material is visible onsite. The contractor stated “Steve [Dyne] has the seed mix and is not onsite today”. Violations cited on 6/21/10 and 3/7/12, Inspection Records #1906058 and #2048796, remain outstanding. No new violations issued during this inspection.

The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed.

2812060 Primary Facility 2018-12-07 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this uncompleted conventional well was conducted December 6, 2018. At the time of inspection, the well has a 7" x 2" candy cane vent pipe 4' high with a marker plated erected atop 7" surface casing as a plugged well monument. There was no visible or audible venting at the vent pipe. No gas was detected at the mouth of the vent with an Altair 5x gas meter. No discharge, venting, or bubbling was observed outside of the 7" casing. No obvious issues were observed during this inspection.
