The operator should continue to monitor the filter socks to ensure that they are continuing to function, until final restoration activites are completed. This is a MARCELLUS WELL.

Subcontractor, BJ was at the site performing set up of the CO2 frac. Multiple tanker trucks of CO2 and Nitrogen along with pump trucks were on-site. Barriers were aligned in an acceptable manner. No Freeboard issue with the pit and Frac water pit was built. Confirmed frac time of 8/18/09 at 7:00 hrs and departed site.

DEP inspected well sign, E&S plan and access road to well head. No Freeboard issues with both pits and pits are aligned with 30 mil plastic. Samson representatives, Charlie Howard and Steve Trujillo. Subcontractor, BJ was at the site to perform the CO2 frac. Multiple tanker trucks of CO2 and Nitrogen along with pump trucks were on-site. Safety meeting was held on-site prior to frac. Test lasted for approximate 30 minutes for nearly 6000 psi before mechanical failure occurred. BJ and Samson were going to attempt a second CO2 frac once the well was purged.
1828962 Primary Facility 2009-09-09 Routine/Complete Inspection No Violations Noted The freshwater pit onsite has been relined and was being refilled with water trucks at the time of inspection. The pit is within freeboard requirements and has no sign of leaks at the time of inspection. The waste pit is still present and containing waste water and cuttings. No E&S issues on-site.

Pits were removed and were partially reseeded. Well pad was rocked, but site had not been entirely seeded and mulched; therefore, 70% vegetative growth was not observed. Operator failed to restore well site within nine months of completion date June 22, 2009 and a violation will be issued. Temporary bmp¿s (filter sox¿s) were still in place and functioning. No accelerated E&S issues were observed at the time of inspection.

Pad in place; no production equipment installed yet; perimeter BMP¿s still in place and functioning adequately; site revegetated with minimum 70% vegetative cover. Access road gated, maintained and no E&S problems noted.
1965626 Primary Facility 2011-04-14 Routine/Complete Inspection No Violations Noted Well actively being plugged. Access road appears stable. All pits have been removed, areas seeded mulched with >70% vegetative growth. Temporary bmp¿s in place functioning, operator may remove upon final, permanent restoration, which under regulation is to be established within nine months from the completion of plugging. No accelerated erosion or sedimentation issues noted on site or access road. Spoke with Tom, of Samson Resources, who stated plans have not yet been decided on how the site will be restored, department reiterated that according to regs >70% uniform perennial vegetative cover or other permanent stabilization measures that are capable of resisting erosion are required for permanent restoration. Operator has plans to utilize an erected monument, which is ok by department. Any agreements on specific restoration plans, are handled between operator and land owner and are considered to be part of lease agreement and are not handled by the department. Follow up inspection will be conducted to ensure permanent restoration.

1996483 Primary Facility 2011-08-04 Routine/Complete Inspection No Violations Noted Site visit was requested by land owner Leroy Troutman. PA DEP O&G Inspector Paul Wyeras was also present.
1997391 Primary Facility 2011-08-04 Routine/Complete Inspection No Violations Noted DEP (Mark Maestera and Paul Wyrwas) met with Mr. Troutman and discuss the site restoration and well monument. DEP confirmed that the monument was completed as per regulation. The site has been restored under 78 regulations and the slope of the site pad can not be regraded due to stabilization of vegation and concern of disturbing the structural integrity of the abandoned gas well.
