0360102433 2015-03-17 Final Restoration No Violations I was on location to check this location for final restoration. The stock tank and the associated piping are still on the location. The stock tank is not properly sealed and leaking small amounts of oil and brine out of an open valve. The electric poles are still in place. I did not pass the Final Restoration at this time. I called Richard Kempthorn and requested the additional reclamation and information regarding why the restoration has not been completed. I was directed to contact Bob Bervin 330-828-8420 (Lake Region Oil) regarding the restoration. He stated he will evaluate the well and will begin the required work weather permitting on behalf of Richard Kempthorn. The stock tank that is on location was used by Republic Steel to produce the well. The tank is too large to transport on the road and will have to be cut up and pieced out before it can be transported from the location.

1645977896 2015-04-27 Final Restoration No Violations I was on location to pass final restoration for this well. The well has been plugged and there are no signs of the well leaking. The production equipment, tanks and power poles for the well have all been removed. The location of the wells is an industrial area and the debris and rubble on location is not associated with the well. There are no violations at this time and final restoration has been passed.
1948532152 2008-11-14 Plug / Plug Back No Violations The service rig will finish pulling the 5 diameter casing today. A stone/brush plug will be set at 2579 and clay will be placed from 2579 up to 2329.
