RBDMS indicated that the wells on site are in producing order.
The production equipment on the pad consists of the following: 14 400 bbl. Production tanks 4 Producing wells 4 Sand separators 4 Line heaters 1 compressor 1 Vapor recovery tower 1 combustor 1 30bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A. |
1298737560 | 2013-04-17 | Casing String Witnessed | No Violations | I am on site today to witness the running and the cementing of the 9-5/8 inch diameter casing string. Patterson Drilling Rig # 480 drilled a 12-1/4 inch diameter hole to 6975 ft and will set casing at 6958 ft . Halliburton Cementing Service will cement casing with 480 sks lead on stage 1 at 14.2 and 490 sks of tail at 15.6 then wait 4 hours and set stage 2 with the DV Tool at 4081 ft and cement with a lead of 700 sks at 15.6 and tail of 300 sks of 14.4 I am on site because they are doing a Fit test prior to cementing this casing string. The fitness test will prove if the deep casing string is needed or if the casing program can be updated. |

This pad contains 4 properly labeled producing wellheads. Production equipment includes 5 400 barrel steel tanks in their own containment, four GPUs, 4 sand seperators, 4 vertical seperators, and a vapor recovery unit. Surface annular pressure is 150 and intermediate annular pressure is 70. Production records are current in RBDMS. |

During my inspection I discovered that they had set 461 ft of 18-5/8 diameter casing. I ask if they had forgot to contact me for the casing cement job and was advised that they had contacted me. Phone records show yes they called to advise of the spud date but had no further contact. While inspecting the site I noticed that the drilling rig and the Producing well are very close to each other. I ask and was advised that the well was also in production at this time. I then contacted Deputy Chief Claus and advised him of my concern. Deputy Claus then advised me to shut the drilling rig down until some safety measures could be installed. I contacted Gulfport Energy and advised of the safety concern. The rig was then shut down and a meeting scheduled to review how things would progress. |

Patterson Drilling # 480 has moved over to this well which has conductor - surface and intermediate casing already set. They are testing the BOP before they knock out the bridge plug set at 1502 ft. The BOP test failed and they are making repairs and will test again later today. After the test passes they will begin drilling. They have added a device to connect to the production tankers. Now they can vent the gas generated while loading the tanker into the flare line and burn off the vapors. I advised that they check the safety burn back valve each time before they load to insure it is working correctly. The check valve is so that the flare line does not burn back to the tanker while filling. |
1391096712 | 2014-01-29 | Production Wells | No Violations | I am on site today to because Chris Anderson called me to inform me of a fire which occurred at the line heater. He said that the line heater caught fire after a gas leak which may have been caused by the pumper not shutting a valve while working on the heater. The fire was contained to the line heater and was put out by the pumper. The well was to be shut in until repairs are made. During my inspection I found that the heater is back in service and the well is back in operation. There is some heat staining on the side of the line heater which was in the fire but no other sign of a fire. |
1406199285 | 2014-07-24 | Production Wells | Violations Noted | I am on site tonight because of a spill which occurred due to line leak on the separator. It allowed condensate under pressure to spray a mist over an area 45 ft x 145 ft. The area affected is next to the production tanks and the fenced in area. The amount is approximately 5 barrels of condensate. Diapers have been put down to absorb any free fluid. The clean up crew will remove any contaminated rock or soil. The area outside of the fence has vegetation and it is to be cut with a weed eater and Bio Sorb put down. The Bio Sorb will be vacuumed up after it has absorbed what ever it can for an 8 hour period. Vegetation will be monitored and replaced if needed. |
1406298623 | 2014-07-25 | Production Wells | No Violations | I arrived today to check on the clean up of the 5 barrel condensate spill which occurred 07/24/2014. I found that the contaminated rock and soil has been removed and clean rock and soil put down and graded. The equipment removed to excavate under it has been put back in place and is being connected at the time of this inspection. None of the wells are in operation at this time and during the shut down period the sand valve which failed due to sand cutting has been checked on the remaining wells to insure it does not occur again. |

When I ask if they had been drilling I was advised that they had not drilled. I had counted the stands in the derrick and now there are 4 more. I contacted Chad Shoffner and he said that yes they are at 600 ft but that the information was incomplete. I told him I had counted the stands in the derrick and that he should be more open when ask such an easy question. He informed me that all of the safety measures would be in place before any drilling continues. |
