0267315253 2017-06-28 Plug / Plug Back No Violations I was on location to witness plugging and abandonment activity being performed by Nicks Well Plugging under contract to the well owner/operator. The bottom formation plug had been set on the preceding day. At the time of this inspection, I inspected the wellhead and found no measurable residual pressure. Crews from Appalachian Well Surveys were on location to shoot the production casing. The remaining plugging activities were executed and completed according to plan and without incident or problem. I conducted continuous ambient air monitoring for the presence of explosive gas and hydrogen sulfide and observed no readings. Upon completion, the top surface plug was stable at grade. Crews cut the well casing to 30 inches below grade as prescribed under 1301:9-11. The former tank battery had been dismantled and the site graded level.

1231709362 2014-08-13 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 09/21/2007. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery in separate board-on-board enclosures. The wellhead and surrounding area are free of signs of leaks or other impacts. The wellhead and battery are located in a residential portion of Gates Mills, adjacent to Mayfield Road in a flat-lying area behind the residence at 1440. Access to the wellhead and battery are via the concrete-paved entry to the residence, and then a short continuation (150) to the west. . The battery is comprised of 1 steel 100-barrel production tank, 1 50-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-out is bull-plugged. The secondary containment at the battery appears well maintained. Process safety equipment is present. I did not observe any signs or evidence of leakage, spills, or problems. The ID placards at the wellhead and battery are in good condition.
1258274925 2013-10-22 UrbanPreliminary Restoration No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 1 140-barrel steel production tank, 1 50-barrel brine tank, Dominion metering run, and separator unit. The battery and wellhead are enclosed in separate board-on-board fences. The load-outs are bull-plugged. The ID and secondary containment are good. The unit is fitted with carbon canister, low-pressure relief valve, and spark arrestor. Gates are locked. Lease road is in good condition and areas impacted by drilling have been restored. No problems noted and passes preliminary restoration.
1740493920 2017-10-27 Final Restoration No Violations I inspected this well location as an evaluation for Final Restoration. Plugging and abandonment was completed on 6/28/17, under witness by an ODNR inspector, and a report filed. At the time of this inspection, the areas of the former location of the wellhead and battery had been re-graded and re-seeded with vegetation established. Final Restoration is complete.
