Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0092426129 2014-11-12 Drill / Deepen / Reopen No Violations I was on location to check the status of Poulson Drilling this well. They are currently drilling on air a 7.875 diameter hole at 1850. They will continue drilling ahead to their approximate total depth of 5350. There are no violations at this time.
0189831237 2015-05-28 Preliminary Restoration No Violations I was on location to check the status of restoration for this well. A contractor was on location performing dirt work and reclaiming the drilling location. A pumpjack has been set on the well today. I explained to the operator that a mechanical pressure relief valve and pressure gauge must be installed on the 8.625 diameter surface casing and production casing annulus to monitor pressures. Flowlines for the well have been installed and are waiting to be covered by topsoil. I will follow up when reclamation is complete to pass preliminary restoration.
0278557963 2018-11-15 Preliminary Restoration No Violations My inspection found the pits have been filled and the land restored around the well and tank battery locations. There are no signs of active erosion at either location. The preliminary restoration is compliant with Ohio Revised Code 1509.072(A). The preliminary restoration is passed.
0399886370 2014-11-21 Preliminary Restoration No Violations I was on location to witness the closing of the pits for this well. A total of two loads of dust per pit have been brought onto location and are being mixed by a Palmer and Sons backhoe. Stallion vacuum trucks are also currently hauling off water from the of tops of the pits. The pits will be closed fully today.
0677761487 2018-11-15 Production Wells No Violations I inspected this well. The well is equipped with a pumpjack. The tank battery consists of two steel production tanks. Two plastic brine tanks, a horizontal separator and a meter. The tank battery is shared with well # 151-25872. The well is legibly identified. No compliance issues are cited at this time.
1415380869 2014-11-05 Surface Facility Construction No Violations I was on location to check the status of Poulson Drilling rigging up on location. Currently the air compressors and a load of drill pipe are on location. Some loads will be moved today but the majority of the move will occur on 11/6/14.
1505111168 2015-05-05 Completion Testing No Violations I was on location to check the status of stimulating this well. Producers Service Corp was currently rigging up on location. The frac is currently behind schedule due to the longer period required to frac the RTB 1 well. The crews will rig up today and complete the job before sunset.
1781164130 2014-11-06 Surface Facility Construction No Violations I was on location to check the status of Poulson Drilling rigging up on location. Currently the rig and all equipment have been brought onto location and are being spotted on the location. The working pit is currently being dug and is composed of a clay soil and is not making any water and is stable. They expect to spud the well in the morning on 11/7/2014.
1908172091 2014-11-07 Casing String Witnessed No Violations I was on location to witness the cementing of the 16 diameter conductor casing for this well. 36 of drift was encountered and it was decided to drill a 20 diameter hole to 64 and install 54 of 16 diameter 65 lb/ft pipe. Universal rigged up and pumped 100sks of type 1 cement with 3% CaCl and .25 lb/sk flake. They displaced the cement with 8.8bbls of fresh water and there were complete returns during the job and 8bbls of cement slurry returned to surface. Poulson Drilling will wait on cement for 8 hours before breaking off and drilling ahead.
