During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of one sand trap, two gas production units, six 500 barrel tanks, one compressor, and two combustors. Identification was present and production records were current.
1449772879 2015-12-10 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found Hall Drilling rig # 5 is now on the hole and has drilled a 24 diameter surface borehole to a measured depth of 391 and ran 360of 20- 94lb/ft J-55 buttress thread casing with a guide shoe and float collar ran along with 2 casing centralizers. Universal Well Service is on location to cement the casing using 585 sacks of Type 1 -15.6 ppg 1.20 yield cement with 3% Calcium added and 25 barrels of Viscous space ran ahead of the cement. The cement was displaced with 112 barrels of fresh water and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns witnessed to surface. The cement mix water was tested prior to cementing to verify PH and chloride levels.

The site is being cleaned up after the service rig drilled out the plugs. AOS is onsite removing standing water from containment. The company representative notified me of intent to flare on Tuesday 7-5-16.
1553668928 2014-10-19 Casing String Witnessed No Violations My inspection today finds Butchs Rathole drilled 100 of 36 hole. Butchs then ran 100 of 30 weld casing. Universal cemented the casing in place with 330 sacks of type 1 cement. A test of the cement mix water indicated that chlorides were less than 100 ppm. I witnessed cement circulate to surface and stay at surface upon completion of the cementing operations.

The 5.5 inch diameter production casing 936 pounds per square inch and the production tubing read 404 pounds per square inch. The production equipment consisted of two high-low separators, six 500 barrel tanks, two compressors, and two combustors. Identification was present and production records were current.
