0262437840 2018-10-24 Production Wells Violations Noted I conducted an inspection on this well. At the time of this inspeciton, the well was not in production, and consisted of an 8 5/8 casing with tubing exposed and a gate valve attached on the tubing. The gate valve was closed. There were no flow lines connected to the well. No production records have been submitted for this well. There was no production equipment associated with this well. Access to the well was through a path in the woods. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. In the interim, the owner must correct the identification sign.
0814496788 2018-09-14 Production Wells No Violations I conducted an order follow-up inspection for Chiefs order #2014-202 for failure to maintain liability insurance. The well was shut--in and consisted of a 8 and 5/8 casing with tubing exposed and a gate valve attached on the tubing. At the time of the inspection, the pressure reading on the gauge showed zero. No other production equipment was associated with this well and the well is incapable of production. This well was drilled in 1999, and production has never been reported for this well. No insurance is on file from Plateau Energy Corp.

At the time of this inspection, the well was not capable of being produced. There was a surface casing with production casing with closed Demco valve. There were no active flow-line connections. The were was a tank battery and separator associated with the Anderson 1 (#2-4076), to which this well may have produced historically. There remained no active liability insurance on file for Plateau Energy Corp.

At the time of the inspection, the pressure reading on the gauge showed zero. No other production equipment was associated with this well and the well is incapable of production at this time. This well was drilled in 1999, and RBDMS shows no production records since the well has been drilled.

The gate valve was shut. There were no flow lines connected to the well. There was no production equipment associated with this well. Access to the well was through a path in the woods. Compliance has not yet been taken care of or a plugging permit obtained.
