At the time of this inspeciton, the well was not in production, and consisted of an 8 5/8 casing with tubing exposed and a gate valve attached on the tubing. The gate valve was closed. There were no flow lines connected to the well. No production records have been submitted for this well. There was no production equipment associated with this well. Access to the well was through a path in the woods. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. In the interim, the owner must correct the identification sign. |
