0535394610 2004-01-29 Orphan No Violations Inspection this day found I/O well, hole appears to have 6.25 surface casing, 4 7/8 production casing with rods and tubing still in well. Sits directly in center of fron yard for trailer house, which is approximately 15 from house trailer where someone lives. Took pictures. 2 old tanks sitting of to side of acerage - probably goes with well but very rusty - may fall apart when picked up. Needs plugged but no hazard. No overhead or underground. Will begin process for I/O status.
0587467505 2004-11-18 Final Restoration No Violations Inspection this day found well has been plugged to surface with cement, location has been reclaimed, seeded and mulched. No complaints from landowner
1082042341 2004-04-15 Orphan No Violations Oil starting leaking from this well on Monday, I was not called until lat Wed. Inspection found idle well had filled up with water and pushed oil out, travelling downhill and under existing trailer down towards Little Muskingham Rv. Did not reach river and was only 2-3 gallons oil, at the most, it is estimated. Most oil ended up under the trailer so difficult to clean and really made odor inside trailer. Level has lowered and is no longer leaking - level ab out 6 below surface at this time. Landowner has put lime down where oil has laid - left additional pads with him in case it happened again until State can get it plugged. Trailer is only 20 below - away - from the well, which has rods and tubing in it. Has not leaked in the past, but heavy rains have filled it up. We complete I/O application and try to hurry thru.
1103742355 2004-12-22 Final Restoration No Violations Inspection found well plotted incorrectly on map - should be where old circle is across the road, not north up on next road where my map is showing - should be corrected as is plugged well - was data not taken off the plugging report, Mike?
