1434981691 2015-06-22 Preliminary Restoration No Violations On the date of inspection I found the site to have been restored to 1509.072 standards and regulations. I passed the preliminary restoration at this time. Well is capable of production and the site is in good shape.
1484725055 2013-02-19 Production Wells No Violations Well equipped with a pumping unit with a gas engine. The valves on the production casing and the tubing were open. Well has good identification.
1569080891 2012-10-10 Annular Disposal No Violations Annular Disposal Mechanical Integrity Test. Jeff Burke tested the annulus three times using nitrogen. All three tests failed to maintain pressure of 204 psi. Jeff Burke disconnected the annular disposal line at the wellhead and is storing the brine in a 100 barrel steel tank.
