2533934 Primary Facility 2016-10-31 Incident- Response to Accident or Event No Violations Noted Michael Williams, Environmental Health and Safety Specialist of SWN contacted PADEP NERO Emergency Response Team in Wilkes-Barre, PA at 10:30 PM on 10/30/16 to report a spill of (3) barrels, (125) gallons of hydraulic fracturing fluid. Bob Bisignani, Assistant Emergency Response Coordinator in NERO contacted the PADEP Scranton District Office at 8:45 on 10/31/16 to report the spill to the Oil and Gas Management Program. The complaint details in the complaint inspection form state the following. "NR4 Brandt Pad lost 125 gallons onto containment of frac water (sand laden liquid) and when cleaning up, a ½ inch hole was discovered in containment. Contractor was called in to clean up. Spill occurred Oct. 30 at 10:30 PM." Mike Deming Senior Regulatory Analyst of SWN contacted me at 9 AM on 10/31/16 regarding the spill and to confirm that the Oil and Gas Program in the Scranton District Office was notified of the release. Mr. Deming stated the following information in reference to the spill during our phone con
2552320 Primary Facility 2017-01-11 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:25a.m., the cellar was about half full of ice. The well was drilled to TD and set up with a night cap but hasn’t been stimulated yet. I took a walk around the location and there were no obvious concerns noticed.

2555019 Primary Facility 2017-01-25 Administrative/File Review No Violations Noted On 1/19/17 at 16:06, Mike Deming, Senior Regulatory Analyst of Southwestern Energy Production Company submitted a report to the Department in reference to an investigation conducted for a potential release of hydraulic fracturing fluid to the Brant (North Range Unit 14) pad surface on 10/30/16. The Department has completed a review of the field notes, lab report and tabulated results of the investigation conducted by SWNPC and has concluded that no further action is warranted of SWNPC at this time. Please see the Department's 10-31-16 inspection report for additional details associated with the release.
2591595 Primary Facility 2017-05-04 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m. for an inactive status request inspection.

The well was drilled to TD at 13,374' but wasn’t stimulated yet. The cellar was about half full of water with no bubbling noticed. There were no lines going to or from the well, all the valves were closed and the pressure gauges had 0psi on them. There was a TA cap on top of the well and a metal guard around the well. There were three wells on site, the 3H (115-22085), 4H (115-21648), and 2H (115-22086). The 2H is the only well that is in production. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:35p.m.

The well was drilled to TD but not stimulated. There were no lines going to or from the well. There was a TA cap on the top of the well. The well was granted inactive status on 5/31/17. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2793276 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Overall, the site has good permanent stabilization. No E & S issues identified. No violations noted.
2804821 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well site was covered with snow. The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1' below grade in the cellar and there was no bubbling noticed. There were 22 empty 500bbl tanks still on containment from the recent frac, the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
