0520392802 2014-08-26 Preliminary Restoration Violations Noted Upon inspection, it was observed that much of the well pad location is void of vegetation and the eastern slope of the pad is allowing significant erosion to take place. In addition, the lease road needs maintained and the slope to the south of the lease road is slumping. Barren areas on the location need to be re-seeded/mulched to encourage stabilization of the soil and minimize erosive conditions. Preliminary restoration cannot be passed until these conditions are met. I will contact Mr. Carl Heinrich via phone call and notify him of this situation. Mr. Heinrich will have until 9/30/2014 to complete the preliminary restoration for this location.
0739945355 2013-08-26 Drill / Deepen / Reopen No Violations Status check on drilling operation with the accompaniment of Mr. Rick Cisler and Mr. Kenny Brown. Total depth at the time of inspection was 2310 ft. Company should be ready to run 4.5 pipe tomorrow. Inspected pit, and walked around site. Everything is in compliance.
0814456268 2013-09-09 Drill / Deepen / Reopen No Violations Logging of this well took place shortly before I arrived on site. The wireline company and Mr. Carl Heinrich were still on site. The top of the cement on the production casing string was at 1760 ft. according to the log. Pit has been closed and the site has been graded.
1277995746 2013-08-14 Drill / Deepen / Reopen No Violations Witnessed the surface casing being ran and cemented. New pipe was used for this string. Good circulation was achieved and good returns were observed. Once cement was displaced, it stayed at the surface and did not drop. Pressure test was conducted in conjunction with the Blowout Preventer (BOP) test. Everything was pressured up to 500 psi and was held for 15 minutes. Mr. Jim Wood with the Bureau of Land Management reported that there was no drop in pressure.
1284451388 2015-05-14 Administrative Inspection No Violations Received call at 9:32 am from Christy Chavez of Heinrich Enterprises. She stated that their Field Manager, Mr. Sam Johnson, has been working with contractors as well as Wayne National Forest to fix the slipping slope above the lease road on this location. She stated that work will be done to fix the slip this summer.

He had cut the ditch in and laid culvert in it. He planned on cutting trees and doing dozer work the next day. Walked back to site, and went over the plan with Mr. Jackson.
1429812888 2015-05-11 Production Wells No Violations Upon inspection, it was observed that the majority of the location is now stabilized with a vegetative cover. The slope above the lease road is still slipping and eroding and has little vegetatation growing. I spoke with Mr. Jeff Jackson who does Heinrichs dirt work and he stated that he has been contacted to perform remedial work on the slip this summer. Sheet pilings may be used to hold the slope in place once it has been fixed.
1618160771 2015-03-02 Production Wells No Violations Upon inspection, it was observed that the area above the lease road is still slumped and needs addressed. In addition, the southeast corner of the location still has deep erosional gullies that need to be addressed. I will contact Ms. Christy Chavez of Heinrich Enterprises and let her know that these issues need addressed as soon as the weather is fit.
1686573044 2013-08-29 Drill / Deepen / Reopen No Violations Rig has been moved off of the site. Mr. Bryan Chavez reported that three loads of fluid had been taken off of the pit and only cuttings were left. Well was shut in with a 4.5 in to 2 in swage and a 2 in valve.
1745878548 2013-08-21 Drill / Deepen / Reopen No Violations Witnessed the cementing of the 7 intermittent casing string. Circulation was achieved during the displacement of the wiper plug. Oil was circulated to the surface. Well was also gassing throughout the process. Accompanied by Mr. Jim Wood from the Bureau of Land Management.
1830512984 2015-10-05 Preliminary Restoration No Violations Upon inspection, it was observed that slumping issues along the lease road for this well have been corrected. Proper drainage, in addition to a ditch, have been installed along the lease road and the overlying slope has been put back to a 2:1 grade. Vegetation is growing on the hillside as well as all of the formerly barren patches along the perimeter of the location. The compliance notice for failure to restore in six months dated 8/27/14 can be closed. Preliminary restoration passed 10/5/2015.
1945822065 2013-08-22 Drill / Deepen / Reopen No Violations Witnessed the BOP and casing test with Mr. Jim Wood from the Bureau of Land Management. After 7 runs with adjustments after each one the well passed the test. It started at 500 psi, and ended at 490 psi after 30 min.
1989120741 2014-10-30 Preliminary Restoration No Violations Upon inspection, it was observed that the slip on the area above the lease road has been worked on recently. The barren areas along the eastern perimeter of the location is still present and deeper rills and gullies have formed due to soil erosion. I will call Heinrich Enterprises and recommend they re-grade this area and stabilize it with mulch for the winter months and re-seed it this spring when vegetation has a better chance of taking root.
