Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1380823598 2013-10-03 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A.
2054724417 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. All wells on location are using the plunger lift system as the method of production. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

10 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Horizontal heater-treater. Equipment present on the location that is not associated with the production equipment consists of: 3 18 Wheelers with trailers 1 track hoe 1 Short bed semi 1 dump truck 1 skid steer 1 power plant 2 pickup trucks

8 small trailers 1 40 foot box trailer 1 16 foot box trailer 3 fuel tanks 35 joints of 9 5/8 inch diameter casing. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
