0871839403 2018-07-09 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has erosion control in place and vegetation established. The said well drilling and completion process is complete. The well is equipped with annular pressure monitoring with 250 psi showing on the 13.375 surface casing gauge and 1,800 psi on the 9.625intermediate casing gauge. The mouse hole from the drilling rig will need filled with cement. Signage is posted on the well head and at the gate with well control shut down devices at multiple points on the well pad. There is 2 gas processing units present at the rear of the well pad supporting each of the 2 producing wells on the pad also present is a production fluid tank battery consisting of a total of 3 steel construction 400 barrel capacity tanks present inside a polymer lined steel containment dike area. The tanks have the dome lids secured and closed with 12 ounce pressure relief devices on the production tanks that are releasing into a 100 barrel tank that is set on location away from the other production equipment, no production tanks are equipped with any low pressure recover system. I discussed with the production man on location identified as Adam Mallet the need to install a low pressure recover system and not allow the vapors or gases to be released into the atmosphere. Also discussed was the lack of a gauge or any type of annular pressure monitoring on the 20 diameter string identified as the surface casing string. Also discussed was the need to release the annular pressure off the 13,375 and 9.625 casing strings.
1384378737 2017-10-19 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling progesss found H&P Drilling rig 611 on location and currently drilling cement out of the inside of the 13.375 diameter casign string at a depth of 2,014. The cement was tagged inside the casing at 1,500 and the job locked up during cement displacement. The casing was set at 2,510 and the company indicated they circulated the well with KCL water prior to cementing. The company will drill out the cement and bond log.
1508860257 2017-10-24 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found H&P Drilling rig 611on location. Historically on 10/23/2017 the company man on location identified as Tim Field notified me that they were currently drilling the 12.250 intermediate well bore at a measured depth of 7,432 and has inquired a influx of brine water measuring 8.5 pound per gallon and 75,000 parts per million chlorides with 27 parts per million H2S. The well blow out preventer was shut in and had 500 psi on the annular of the drill sting. On this day upon my arrival the company man Tim Fields informed they were preparing to open the well up and see if it was still would open flow with a 50 barrel maximum allowed limit. The well then flowed the 50 barrels and H2S was present. The company indicated they were preparing to pump 10 pound per gallon Diesel fuel oil based mud down the well to maintain well control and finish drilling the remainder of the well bore. I then spoke with Deputy Chief Dave Claus and we requested and implemented a conference call with Triad and requested they not drill the well with Diesel Oil Based drilling mud.
1509469995 2017-10-31 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 611 on location and fluid drilling the 12.250 2nd intermediate well bore at a measured depth of 8,626. The company is drilling with 8.9 pound per gallon diesel fuel based drilling mud maintaining well control using managed pressure drilling with 242 psi on the 13.375 casing well head section and 2,150 psi on the drill string. They have a PDC bit in the hole and are in the Rose Hill formation with the Packer Shell coming up at approximately 8,800. No H2S gas is currently present.
1509985188 2017-11-06 Drill / Deepen / Reopen No Violations Status check on the well drilling process found H&P Drilling rig 611 on location and the rig has currently completed the total drilling depth of the 12.250 2nd intermediate well bore at a total measured depth of 10,275. Upon reaching total depth the well bore was conditioned before pulling the drill string and at the time of my inspection the well bore was static with 9.5 pound per gallon diesel based drilling mud in the hole. The company did run a caliper log upon completing the drilling process. The rig is currently running in the hole with the 9.625 casing string. There is a TAAM company swell packer and DV tool on the string that will be situated at the top of the Bass Island formation once the casing string is fully inserted into the well bore.
1510076573 2017-11-07 Casing String Witnessed No Violations
1861642814 2016-02-10 Drill / Deepen / Reopen No Violations The said well was spudded on 02/28/2014 with 100 of conductor being set. The well was not completed past that point and the drilling permit is set to expire on 05/01/2016.
1897291644 2017-01-24 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site has earthen slips on the East and South West side fill slopes along with a slip below a dirt stockpile on the South East slope. The slip on the South West slope breeched the limits of disturbance depicted on the original as built well plans. The slips have not been repaired. The well pad is in violation of ORC 1509.072. The well was spudded on 02/28/2014 and 20 diameter conductor casing was set to 100 and no further well drilling was completed past that point. No production equipment is situated on the well pad. The drilling permit expired on 05/01/2016.
