Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units (GPU), (3) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 950 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 460 psi. |

During my inspection Patterson Rig 803 had reached total depth at 20244 feet and had 5.5 inch diameter production casing set at 20163 feet. Prior to my arrival BJ Service had just completed the cementing of the 5.5 inch diameter casing. I obtained the post cement job report and discussed with the CNX company man their timeline for walking, spudding, and cementing surface/mine string casing on the Switz 7 D well. The rough estimate for cementing the surface/mine string is Thursday night. The coal seam will be at 595 feet to 604 feet. |
1517770241 | 2018-02-04 | Completion Testing | No Violations | On this day I received a call at 10:59 am from ODNR Emergency Response member Nathan Lane who indicated the company had reported a fluid release on site from the completion flair stack. I arrived on site at 11:10 am and contacted the Company man on site who indicated they had recovered any material affected from the release. It was observed that a area directly below the TETRA Company flair stack well control device had been excavated and loaded into a roll off container with what appeared to be approximately 1.5 yards of mostly limestone gravel inside the container. There was a polymer liner in the container however no standing fluid or sheen could bee seen inside the container. The area of excavation was also checked and no standing fluid or sheen could be seen. There was a well pad perimeter drain on site near the release and was in the closed position. Addressing the well status, the well drilling and stimulation process is complete. The completion plugs have been drilled out and production tubing hung in the well. TETRA flow back services are on site and rigged up to the said well, they will begin the initial well bore production flow back on this day. |
2058318159 | 2017-05-25 | Casing String Witnessed | No Violations | On site inspection to witness the conductor casing cement job with Express Energys top hole rig and cement service. The auger rig has drilled a 36 diameter borehole to a measured depth of 100 and inserted 100 of 30 diameter .375 wall 118 pound per foot conductor. Express then cemented the casing with a 48.9 barrel cement mix consisting of 275 sacks of class A cement with the cement weight verified by weight of scale at 15.6 pounds per gallon. Cement returns were witnessed to surface inside the cellar at 58 barrels into a total of 75 barrels of fresh water dissplaceent.1 of cement was left in the cellar causing a liquid tight seal. No fresh water was encountered in the borehole while drilling. |
