0000000537 2002-10-30 Production Wells Violations Noted See Case # 02-044. 10-29-02 Received a complaint from Reggie Brown (OEPA; Work 330-963-1137, Cell 216-789-9280) regarding a line leak at a construction site located at 846 3rd St. in Massillon, OH. 10-30-02 Inspected the leak area and met with Jason Lattur of Scheetz Building Corp., North Canton, OH, Cell 330-760-2318. Lattur stated that the release was cleaned up on 10-29-02 by Sun Hazmat Team and that Tom Huffman (Cell 740-680-8648), a well tender for North Coast Energy, assisted in locating the well. Inspector Claus, Jason Lattur, and Tom Huffman located the well and tank battery on 10-30-02 and shut the well in and shut off power to the pumping unit. The current I.D. sign on the stock tank said Environmental Exploration 330-497-4848. This number rings into a fax line. The phone number on the surety list, 330-497-9459, rings but nobody answers. An attempt will be made to locate a responsible person from Environmental Exploration. RBDMS showed no reported production for the well since 1997. THE WELL APPEARS IDLE. PLUG or PRODUCE.

0253436437 2004-05-04 Production Wells No Violations N.O.V. has been resolved. Well is in production. Contact Bo Blum (330-418-6550); place a new I.D. sign at the well.
1394461070 2014-03-24 Final Restoration No Violations During my inspection I found the former location of this well to have been restored. The lease road is now a commercial drive for a wireless tower. Final restoration can be passed at this time.
